- Health Fitness Educator 96th Contracting Squadron, Eglin Air Force Base FL intends to award a Firm-Fixed Price contract for services of a Health Fitness Director. The place of performance is Eglin Air Force Base FL. This is a combined synopsis/solicitation for services in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. The Government reserves the right to cancel this combined synopsis/solicitation either before or after Request For Proposal (RFP) closing, with no obligation to the offeror by the Government. The announcement number is F1TECG5299A100. The North American Industry Classification System (NAICS) code for this acquisition is 561990, with a small business size standard of $6 million annually. If you are a business, please identify your business size in your response based upon this standard. Qualified applicants must submit the following: a resume that reflects a minimum of three year?s experience working a Health or Wellness Center, or a similar organization [contractor experience working with military programs and procedures is preferred]; a copy of a current Basic Life (BLS) certificate; a signed consent for release of information form; and a certificate of current physical examination [current as of 12 months prior to application]; and proof of award of a Bachelor?s Degree in one of the following fields: Fitness and Health, Sports, or Leisure Services ? with Fitness emphasis, or a Health Science Degree. Contractor is required to undergo a mandatory criminal background check, and must be able to understand, read, speak, and write English fluently. Contractor shall meet all requirements in the Statement of Work (SOW) concerning certifications, experience, skill, health requirements, and the description of services. Evaluation will be based on technical capability and price. See attachments for more detail. Contract period consists of the base (1 Jan 06 to 30 Sep 06) and four succeeding option years (1 Oct to 30 Sep).
