- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis-solicitation is issued as a request for quote (RFQ) W91248-06-Q-0013. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-6. The NAICS code is 334511 and the small business size standard is 750 employees. Solicitation is unrestricted. The solicitation is issued as a Brand Name or Equal procurement for the following item: UxValidator Stab ilator Test Sets with SAS1 and SAS2 systems, NSN: 4920-01-529-3346, Ultrax Part No.: 05-330-07, quantity of four (4) each, delivered to Ft. Campbell, KY. Quotes are requested on FOB Destination basis. Delivery shall be not later than 45 calendar days af ter receipt of order. The provision at 52.212-1 Instructions to Offerors Commercial Items, is applicable to this solicitation. The Government intends to make a single award for all items to one vendor. The provisions at 52.212-2, Evaluation Commercia l Items and 52.211-6 Brand Name or Equal are applicable to this acquisition. If offering an Equal item, vendor shall submit Descriptive Literarture. The Contracting Officer will evaluate Equal products on the basis of information furnished by the off eror. If an offeror has not completed the annual representations and certifications at the ORCA website, offerors are to complete paragraphs (b) through (i) of FAR Provision 52.212-3, Offeror Representations and Certifications Commercial Items and submi t with their offer. The contract clause at 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition.
