Description
This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. Solicitation RFQ 548-06-406. This solicitation is issued as a Request for Quotation (RFQ). This solicitation is issued as full and open competition. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-06. . The associated NAICS # is 325412 with a size standard of 750 employees. The VA Medical Center located at 7305 N. Military Trail, West Palm Beach, Florida, has a requirement for delivery of radiopharmaceuticals. The contract period shall be January 1, 2006 through March 31, 2006. Work Schedule: The Contractor shall be required to furnish services during normal VA business hours (8:00 am - 4:30 pm), including (Federal) national holidays. National (Federal) holidays are: New Year?s Day, Martin Luther King Jr. Birthday, President?s Day, Memorial Day, 4th of July, Labor Day, Columbus Day, Veterans? Day, Thanksgiving Day, Christmas Day. When one of the holidays falls on Sunday, the following Monday will be observed as a legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a national holiday by U.S. Government Agencies. Also included would be any day specifically declared by the President of the United States of America as a national holiday. Services shall be provided in accordance with the following special requirements, qualifications and Statement of Work: QUALIFICATIONS: Contractor must possess Florida license for radiopharmaceuticals.. Contractor shall be compliant with relevant regulatory standards with respect to general environmental safety including, but not limited to, Office of Safety and Health Administration (OSHA) and all JCAHO standards. REQUIREMENTS: The Contractor must provide written information when submitting their RFQ, regarding the following areas: * Guarantee delivery time for both normal work days and on weekend, nights and holidays * Alternate source capabilities * Product experience * Policy for lending of survey equipment * Procedures for delivery during emergency situations * Radiopharmaceutical quality assurance program * Radiopharmaceutical quality control program * Continuous quality improvement program * Emergency procedures for spills, sealed source loss or breakage * Radiopharmaceutical inventory and disposal control system * Exposure survey equipment calibration * Continuing education programs offered * NRC certification and/or policies demonstrating equivalent compliance. Successful quoter must be registered with the Central Contractor Registration (CCR). You may register in CCR on-line at http://www.ccr.gov. Quoters are required to submit sufficient evidence they have adequate equipment, facilities, personnel and the necessary experience to meet the requirements of this solicitation. Quoters shall furnish for evaluation purposes, the name, address telephone and fax number of three companies for whom the same services/supplies have been provided. Failure to furnish this information may result in your firm being determined to be non-responsible to the requirements of this solicitation. STATEMENT OF WORK: Contractor shall supply and deliver radiopharmaceutical requirements for VA Medical Center, Nuclear Medicine Department, West Palm Beach, Florida in accordance with all terms, conditions, specifications and this Statement of Work. The VA Medical Center, West Palm Beach requires the supply and delivery of radiopharmaceuticals in unit dose form for Nuclear Medicine diagnostic procedures. The Contractor shall be able to make a minimum of two (2) scheduled daily deliveries during the normal work week (Monday through Friday), with more delivery times as needed. (Scheduled deliveries will be coordinated between Contractor and Contracting Officer?s Technical Representative (COTR) ) Contractor must also be able to supply radiopharmaceuticals, upon request, AFTER normal business hours (4:30pm ? 8:00am local time), Monday through Friday, 24 hours per day on the weekends and Federal Holidays, as listed above. Physician?s requests for Nuclear Medicine Studies are usually made during normal business hours, 8:00 AM to 4:30 PM (local time). Upon notification by COTR, Contractor shall deliver overnight. For emergent physician needs, Contractor shall deliver within one and one-half (1-1/2) hours of receipt of order. Contractor shall: * Provide a nuclear medicine management & software system with training and support. This will include all software upgrades as they occur. * Abide by (CRF 10) Title 10 Code of Federal Regulation, which states: A licensee may use for medical use only: (a) Byproduct material manufactured, labeled, packaged, and distributed in accordance with a license issued pursuant to the regulations in Title 10, Code of Federal Regulations, Part 30 and SS32.72, 32.73 or 32.74 of this chapter or the equivalent regulations of an Agreement States: (b) Reagent kits that have been manufactured, labeled, packed, and distributed in accordance with the approval by the Commission pursuant to S32.73 or an Agreement State under equivalent regulations for the preparation of radio pharmaceuticals for medical use . * Follow all Occupational Safety and Health Administration guidelines. Contractor must have Nuclear Regulatory Commission certification and/or policies demonstrating equivalent compliance with such standards. Contractor must meet all Department of Transportation, F.D.A., and all other agency rules and regulations (Federal and State). * Perform all quality control procedures as required by Federal and State regulatory agencies. * Provide a copy of their laboratory?s current quality control manual and must be willing to discuss any discrepancies in lab results which do not fit the clinical picture. * Replace the wrong delivered item at no cost to the Medical Center. Contractor must replace any expired cold kits at no charge to the Medical Center. Label all supplies as to the amount of radiopharmaceutical, preparation time, expiration time, date, etc. * Provide an invoice once a month to the Nuclear Medicine Service, showing period covered, billing date, name of preparation, quantity and amount. * Provide upon request a detailed description of the test to be performed. as provided by the Manufacturer. * Provide, at no charge, periodic testing of quality control results upon request. * Assign an individual sales representative to aide in facilitating a smooth coordination between the Medical Center and laboratory pharmacy. * Provide material safety data sheet binders and updates on all delivered products. * Provide patient orientation education video presentations and other appropriate material as deemed necessary by the hospital. * ALL packaging of radioactive material MUST BE clearly labeled RADIOACTIVE on the outside of the carton it is shipped in. * ALL products are to be delivered to the following address: VA Medical Center 7305 N. Military Trail West Palm Beach, Fl. 33410 Attn: Nuclear Medicine Service.
