Description
CONSTRUCTION SCOPE OF WORK FOR ROOF REPLACEMENT DORMITORY 303 AT THE EARLE C. CLEMENTS JOB CORPS ACADEMY MORGANFIELD, KENTUCKY I. GENERAL The intent of this Scope of Work is to define the Construction requirements for: A Roof Replacement for Dormitory Building 303, per the scope of work requirements included in this document. II CONSTRUCTION REQUIREMENTS A. CODES The contractor will conform to all applicable construction codes, ordinances, and other regulations which are enforced by City, County, State or relevant Federal agencies. Where such codes are not in effect, it will be the responsibility of the Contractor to consult and use to the extent practicable, the national building code generally used in the area. OSHA regulations shall apply when applicable. All conflicts and requests for interpretation or clarification shall be submitted to the Earle C. Clements Job Corps Academy. The U.S. Department of Labor (DOL), requires that all construction requirements adhere as closely as practicable to the current edition of the National Electrical Code and the National Fire Protection Association?s (NFPA) Standard No. 101, Life Safety Code. In addition, references and standards referenced with NFPA No. 101 will be considered part of the Life Safety Code and will serve as minimum requirements when applicable. The contractor will not submit plans or specifications to any local or state authority without the prior approval of the Earle C. Clements Job Corps Academy. B. SPECIFIC CONDITIONS 1. All companies submitting bids for this project shall have no less than five consecutive years in the practice of turnkey design-build work of this type, as described herein. 2. Prior to submitting the cost proposal, the contractor shall visit the site and become thoroughly familiar with all pertinent conditions that are included in this Scope of Work. Failure to do so shall not relieve the bidder from the responsibility of properly estimating the difficulty or cost of successfully performing the work. Arrangements must be made with the Academy before visiting the site. 3. A mandatory pre-quote walk-thru is required of all interested Contractors. The mandatory pre-quote walk-thru is scheduled for December 16, 2005, 10:00 a.m., Administration Building 2435. At this time, questions related to the areas of work can be presented and discussed with a representative from the Earle C. Clements Job Corps Academy. Quotes are to be received no later than 3:00 p.m. C.S.T., December 30, 2005 in the Administration Building 2435. 4. The Contractor will be required to verify all existing conditions and inform the Academy immediately of any discrepancies and/or unforeseen conditions which may affect the work. 5. All materials, finishes and hardware shall be selected for their ease of maintenance and high durability. 6. The Academy shall remain in operation throughout the construction. All construction activity shall be in coordination with the Academy in order to minimize disruption of Academy operations. 7. The successful bidder shall submit, within seven days of his notification of the contract award, a schedule of values, and a progress schedule to the Academy. 8. The Contractor and all persons under the contract: a. shall observe security regulations in effect on the academy; b. shall not be permitted to utilize the food service facilities at the Academy without prior purchase of a meal ticket from the Finance department. c. shall not fraternize with the students or staff at the Academy. d. shall not consume or transport any alcoholic beverages, drugs, tobacco products, (Academy is a tobacco free facility effective 10/1/05) or firearms onto the Academy. Persons violating the above shall be subject to disciplinary actions as set forth by the Academy. 9. Each bidder shall submit a sealed bid based upon Part V. - Extent of Work, of this document. A bid analysis will be performed prior to the award of the contract to the successful bidder by the Academy. 10. The Contractor must include with his bid certification that he will conform to the latest Davis Bacon Rate (www.access.gpo.gov/davisbacon) decisions for the area. Prior to starting any work, the Contractor will submit to the Academy a performance bond for 100% of the contract, and payment bond for 50% of the full amount of the contract. The Contractor shall also be required to show proof of the required insurance, such as Builders Risk, Automobile, Liability, and Workers Compensation. 11. The Contractor shall be required to (a) commence work under this contract within 14 calendar days after the date the Contractor receives the Notice-to-Proceed, (b) prosecute the work diligently, and (c) complete the entire work, ready for use, not later than 30 calendar days after receipt of Notice-to-Proceed. The time stated for completion shall include final clean-up of the premises. 12. If the Contractor fails to complete the work within the time specified in this contract, the Contractor shall pay Liquidated Damages, for each day of delay, the amount of $ 500 , paid directly to the Academy. 13. The Academy will retain 10% of each progress payment made to the Contractor, to be placed in a Retention Account, and paid to the Contractor as a final request for payment, following the formal Contract Closeout. 14. The Contractor shall be responsible for the security of materials stored on the site, as applicable. 15. The Contractor shall be responsible for all cutting and patching incidental to the work described herein and shall bear the expense of redecorating, repair, or replacement due to damage caused because of his fault or error. 16. The Contractor shall maintain a clean and safe work area throughout the period of construction. The Contractor shall secure the work area to ensure the protection of the Academy occupants whenever they can readily gain access to the work area. Clean-up and removal of all debris and refuse shall be on a daily basis. The Contractor shall be responsible for the disposal of debris in the Earle C. Clements Job Corps Academy?s landfill. In addition, the General Contractor must secure the work area, preventing dust and dirt from entering and contaminating the habitable areas. Any and all damage to the interior or exterior of the facility caused in the performance of the work shall be repaired by this Contractor at no additional cost to the Academy. 17. Quality Assurance a. Contractor Qualifications: Each Bidder must provide information concerning projects that the contractor has completed, which are similar in nature to the one proposed, including location and persons to be contacted. 18. Upon receiving final acceptance of the project by the Academy, the Contractor shall submit to the Academy all warranties, guarantees, and any other information that relates to the project. C. SUBMITTALS The Contractor shall submit to the Academy for approval, and prior to the commencement of any work, a materials list, shop drawings, samples, specifications, and manufacturer?s installation recommendations, to show compliance to contract requirements for all work elements. D. CONTRACT CLOSEOUT Prior to the final payment, the Contractor shall submit at the completion of work, in triplicate, to the Academy: 1. A notice of final completion; 2. A letter certifying completion of all work; 3. As-built record of materials and installation including wiring diagrams; 4. A certification of one-year comprehensive general liability insurance; 5. Warranties 6. The Contractor, sub-contractor(s), and major supplier(s) Releases of Liens; 7. A certificate of payment to subcontractors and suppliers; 8. The consent of surety to final payment; and 9. The final pay request, including retention. E. SUBSTANTIAL COMPLETION The Contractor shall notify the Academy in writing at least seven days prior to the estimated date of substantial completion for the project, and request a substantial completion walk-through. The purpose of the walk-through is to develop a punch list of items, and request a final acceptance inspection. F. FINAL INSPECTION The Contractor shall inform the Academy at the completion of the work including all punch list items, and request a final acceptance inspection. III. PROJECT SCOPE OF WORK A. MOBILIZATION Furnish all labor, equipment, and tools to cover the on-site requirements to accomplish the work as specified herein. B. ROOFING-BLDG. 303 Furnish labor and equipment to remove shingles. Debris to be hauled to Academy landfill. Furnish labor, equipment and materials to install 25 year shingles, 30 lb. felt paper, aluminum roof starter and flashing. Install new vinyl siding at gable ends to match Dorm 305. Install new aluminum fascia cover and vinyl soffit. Install new aluminum guttering and downspouts. Colors and samples will be verified by Academy personnel before start of job. END OF SCOPE OF WORK IV. ATTACHMENTS A.
