Description
Sources sought to provide Home Oxygen services as detailed below. The VA is seeking businesses interested and capable of performing Home Oxygen services as detailed below. Please respond via e-mail ONLY. Your response should identify your company name, point of contact with contact information, capabilities statement and identify your business size, and if you are (by the definitions in the FAR) any of the following: Small Business, 8(a) disadvantaged, disadvantaged, woman-owned, veteran-owned, service disabled veteran owned or HUB Zone certified. Also state if you currently have a Federal Supply Schedule Contract or a VA contract. Only this minimal information is requested. A Request for Proposals shall be issued at a later date. The NAICS code for the procurement is 532291 and the small business size standard is $6 million. Contractor shall provide but not limited to: complete home oxygen services at beneficiaries_ residence for the Department of Veterans Affairs Medical Center, 4815 N. Assembly Street, Spokane, WA 99205, in accordance with the terms and conditions herein for the period of 1 year plus 4 additional 1 year periods. The Government may elect to award multiple delivery order contracts. Services will include but not limited to equipment, initial set-up of equipment, pick-up and cleaning of equipment, Servicing of equipment per manufacturers specifications, supply of consumable parts and supplies, Rental of equipment, Refilling oxygen cylinders/reservoir, Instruction to beneficiary or family member on the care and us of equipment. The successful contractor (s) must be able to provide services to veterans in the following areas: 1. Eastern Washington (Okanogan, Douglas, Grant, Ferry, Lincoln, Adams, Stevens, Spokane, And Whitman counties) 2. Northern Idaho (Boundary, Bonner, Kootenai, Benewah, Latah and Shoshone counties) 3. Western Montana (Lincoln and Sanders counties). Responses are requested no later than 3:00 PM PDT December 12, 2005 to brian.thomas3@med.va.gov.
