Description
Solicitation for the possible replacement of Intravenous and Irrigation Solutions for TRICARE Puget Sound and Alaska. These products include but are not limited to a complete product line with a variety of solutions and volumes. Those companies int erested in a possible Regional Incentive Agreement (discounted DAPA pricing based on volume) with TRICARE Puget Sound and Alaska must respond to the following questions: 1. Does your company carry a full product line of Intravenous and Irrigation solutions, different types and volumes? Describe your product lines. 2. What are the standards by which your products are manufactured? Comply with NIOSH, OSHA and industry standards? 3. Does your company have a distribution agreement with Cardinal Healthcare? 4. Does your company currently possess, or in the process of obtaining, a Distribution and Pricing Agreement (DAPA)? 5. Does your company have the ability to service to the entire TRICARE Puget Sound and Alaska Region (WA and Alaska)? Please provide your companys distribution network. 6. Can your company provide customer support (1-800-numbers and product representatives for each account)? 7. Does your company provide educational programs and clinical support? Describe material available and type of clinical support (brochures, video tapes, interactive access to web or other). Will product in-service and education be available upon reque st after product implementation throughout the length of a possible agreement? 8. Describe your companys return goods policy. 9. Describe your companys proposed implementation plan if you were to enter into an agreement with TRICARE Puget Sound and Alaska. 10. Does your company provide volume discounts? 11. How does your company recommend disposal of your containers? Does this method comply with regulations to minimize impact on the environment? 12. Does your company have a disposal program? If so, please describe your program and its cost. 13. Provide a history of back orders and/or recalls for this product group with dates, duration, cause, how information was communicated to the customer, interim measures and final resolution. 14. Does your company distribute or manufacture your product line? If you distribute, what brand(s) do you distribute? General Product Criteria for both Intravenous and Irrigation Solutions: 1. Does your company carry latex-free products? 2. What material(s) are your containers made of? (a) Is this material incompatible with any medications? (b) Does this material leach out into the solution under certain conditions? 3. Can your solutions be heated or cooled without chemical and physical alteration? 4. What is the shelf life of your solutions? 5. Do you use non-toxic, color fast dyes for labeling. 6. Are your products containers optically clear? 7. How does your company ensure identification of tampering with a sterile container? 8. Does your company provide sterile solutions, IV and Irrigation, as well as sterile solutions that can be delivered to a sterile field? 9. What type of labeling and packaging is used to clearly identify solutions and avoid confusion? 10. Does your company provide containers that do not react to extreme heat and cold? 11. Does your company provide products that are compatible with all types of tubing? If no, explain. 12. Have there been safety alerts issued by any governmental or accreditation agencies regarding the safety of your any of your Intravenous and Irrigation solutions or the containers used for these product lines? Additional Product Criteria for IV Solutions: 1. Does your product contain DEHP? 2. Does your product contain PVC? Additional Product Criteria for Irrigation Solutions 1. Can your irrigation solutions be connected to IV tubing 2.
