- The GLAC (Great Lakes Acquisition Center) is soliciting quotations for Breast Imaging Services for beneficiaries of the Department of Veterans Affairs Outpatient Clinic at Chicago Heights Community Based Outpatient Clinic (CBOC), 30 East 15th Street, Suite 207, Chicago Heights, Illinois (a division of the Jesse Brown VA Medical Center, Chicago, IL) for the period of January 1, 2006 through December 31, 2006, with four (4) one-year options that can be exercised at the VA discretion, in accordance with all specifications, work statement, terms and conditions contained therein. Offer must be within 12 miles of the Chicago Heights CBOC. All qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their proposal. Offers shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. This is a Firm Fixed Price contract and this acquisition is unrestricted. The size standard for North American Industry Classification System (NAICS) code 621512 and $11.5M. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
