- It is the intent of the Government to award an Indefinite Delivery, Indefinite Quantity Consignment contract for Vascular Surgical supplies used in Cardiological Procedures at the Louis Stokes Cleveland VA Medical Center, 10701 East Blvd., Cleveland, Ohio 44106.. The contract period shall be from date of award through September 30, 2005 with the Government having the option to exercise four one year renewal periods through September 30, 2010. Ordering requests will be made by the COTR on an as-needed basis. Consignment shall include the following items: (1) Non-sealed polyester-knited minicrimp; (2) Non-sealed polyester-cooley knitted; (3) Non-sealed polyester, cooley knitted double velour; (4) Non-sealed polyester-cooley woven low-forosity; (5) Hernashield knitted double velour fabric; (5) Hernashield platinum finesse ultra-thin, knitted cardiovascular patch: (6) Surgical mesh-polyester tape (umbilical tape)-non-sterile; (7) Surgical mesh-polyester tape (umbilical tape)-sterile; (8) Surgical mesh-Lars; (9) Surgical mesh-Trelex Natural; (10) PTFE felts; (11) PTFE pledgets; (12) Hernashield gold knitted double velour-straight tubes; (13) Hernashield gold knitted double velor-axillo bifemoral; (14) Hernashield vantage-woven single velour; (15) Thin wall-straight; (16) Thin Wall-straight externally supported; (17) Standard wall-step centrally supported; (18) Microvel knitted double velour-bifurcations; (19) Microvel knitted double velour-straight tubes; (20) Hernashield gold knitted double velour-straight tubes: (21) Hernashield platinum woven double velour-bifurcations; (22) Cooley veri-soft woven-bifurcations; (23) Woven double velor-bifurcations; (24) Hernashield platinum woven double velour-straight tubes; (25) Intra-aortic-straight tubes; (26) Woven double velour-straight tubes; (27) Aortic arch ?HWDVP 4 branch.
