- Description
- The Social Security Administration requires contractor services to provide physician services, inlcuding non-emergency, emergency and special services, for Federal employees and visitors at the Harold Washington Social Security Center (HWSSC), 600 West Madison Street, Chicago, Illinois, or at any off-site location within the Chicago metropolitan area, as directed by the Government. Contractor shall have daily contact with contractor provided health unit nurses, agency management staff, union representatives, and Government employees. Contact with other contractor personnel and visitors will be limited to emergency services only. Contractor must, at its own expense, provide adequate liability insurance for the duration of the contract period. A current, valid and unrestricted license to practice medicine in any of the 50 United States, the District of Columbia, or Puerto Rico is required. Solicitation SSA-RFQ-01-0016 will be available on August 1, 2001. Requests for copies of the solicitation should be addressed to: Social Security Administration, Regional Contracting Office, MOS/CMR/FMT/CPT, 10th floor, 600 West Madison Street, Chicago, IL 60661, Attn: Nestor Floresca, Contract Specialist, telephone number 312-575-5514, fax number 312-575-4156. Quotations must be received by the Regional Contracting Office no later than 3:00 PM, Chicago time, on Friday, August 17, 2001. This acquisition is a total small business set-aside; quotations from other than small business concerns will be rejected. All qualified sources may submit a quotation.
