- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only soliciation; quotations are being requested and a written solicitation will not be issued. Solicitation number SSA-RFQ-00-0010 is issued as a Request For Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-18. The following provisions and/or clauses apply to this solicitation: 52.212-1, 52.212-3, 52.212-4 and 52.212-5. The Social Security Administration, Harold Washington Social Security Center (HWSSC), 600 West Madison Street, Chicago, IL 60661, is seeking quotations for the acquisition of 180 Exide Model ES-13B (or equivalent) 12 volt batteries, lead/sulfuric acid filled, specific gravity of 1.240 - 1.260 @ 25 degrees C (77 degrees F), vented with clear plastic case, maximum size of 393.70 mm (15.5") high, 120.70 mm (4.75") wide and 266.70 mm (10.5") long. Battery size may not exceed the maximum size stated. A minimum warranty of 15 years is required for any replacement batteries offered. Delivery of the 180 replacement batteries shall be FOB Destination. The contractor will be required to pickup and dispose of 180 unserviceable batteries located at the HWSSC. The unserviceable batteries at the HWSSC are 10 year old Exide batteries under a 20 year warranty. If providing Exide Model ES-13B batteries, the contractor should quote a pro-rated price that includes an exchange of the unserviceable batteries. If providing other than Exide Model ES-13B batteries, contractor must provide the make and model of battery offered, as well as the size and specifications. Quotations shall include the following: (1) price of 180 new Exide (or equivalent) batteries, FOB Destination; (2) price of pickup and disposal of 180 unserviceable batteries; (3) shipping date; and (4) warranty period. All quotations are due no later than 3:00 pm (Chicago time) on Tuesday, September 19, 2000. Quotations will be accepted either in writing or by facsimile. Written quotes shall be submitted to: Social Security Administration, Regional Contracting Office MOS/FMT, 600 West Madison Street - 10th Floor, Chicago, IL 60661. Quotations submitted by facsimile shall be sent to 312-575-4156. Please reference solicitation number SSA-RFQ-00-0010 on all submittals.
