Description
This solicitation will be amended for the following: 1) To change "Brand Name" to now read "Brand Name or Equal". 2) To include the following statement: All products being submitted as equal must have the following salient characteristics: Functional Needs of the USAF Academy Audiological Testing Booth The functional needs of the Audiological Testing Booth are as follows: a) Single wall control/double wall exam rooms. Exam rooms to have double wall 4? panels Type 2 b) Separated by 4? air Space. c) Isolated 4? thick acoustical floors on both control and exam rooms. d) Acoustically engineered independent ventilation mounted external to wall or roof panels. e) Jack panel with cable pass-thru from control room to exam room. f) Recessed incandescent lighting. g) Dual switches for fans/lights in each side h) Passive control room exhaust i) Acoustic door with cam-lift hinges, 32? clear opening. j) Window, double glazed, laminated, safety glass, 30? by 24?. k) Electrical service via a 20 foot grounded power cord. l) True double wall construction, which meets and exceeds necessary attenuation for hearing m) Testing in accordance with ASTM, ANSI and ISO Standards. n) Control side must be able to accommodate a table for the audiometer; cd changer for speech and o) Auditory processing testing; a chair for the Audiologist; as well as another side table for testing forms. p) Exam side must be able to accommodate 2 chairs; the audiologist, a pediatric patient and a parent for q) Play audiometry; wheelchair-bound patients must also be able to have access to the booth; it must also r) Accommodate a table for the tympanometer and otoacoustics emission testing equipment. Must also s) Accommodate play audiometry wall-mounted toys. t) Electrical outlets in each side ? 2 outlets each to plug in otoscopes, tympanometer, otoacoustics u) Emission testing equipment, etc. 3) Offers are now due by 4:00pm EST on 11/13/05 4)Estimated Delivery Date to be December 15, 2005 All other descriptions, terms and conditions remain in effect.
