Description
Solicitation for the possible replacement of Sharps Containers for TRICARE Region Southwest, TRICARE Puget Sound and Alaska and TRICARE Region Pacific. These products include but are not limited to a complete product line and a variety of sizes; ch emo waste, large volume requirements, wall mounts, table top models and floor models. Those companies interested in a possible Regional Incentive Agreement (discounted DAPA pricing based on volume) with TRICARE Region Southwest, TRICARE Puget Sound and Al aska and TRICARE Region Pacific must respond to the following questions: 1. Does your company carry a full product line (chemo waste, large volume requirements, 8 gal., 12 gal., and 18 gal. etc) Provide a list of products provided by your company. 2. Is your company a manufacturer or a distributor? If you are a distributor what manufacturer(s) do you distribute? 3. Does your company have a distribution agreement with Cardinal and Owen and Minor? 4. Does your company currently possess or in the process of obtaining a Distribution and Pricing Agreement (DAPA)? 5. Does your company have the ability to service each Region (TRICARE Puget Sound and Alaska - WA and Alaska; TRICARE Region Southwest California and TRICARE Region Pacific Hawaii, Guam, Korea and Japan)? Provide your companys distribution network 6. Can your company provide customer support (1-800-numbers and product representatives for each account)? Provide numbers and names. 7. Does your company provide educational programs and clinical support? Describe material available and type of clinical support (brochures, video tapes, interactive access to web or other). Will product in-service and education be available upon reque st after product implementation throughout the length of a possible agreement? 8. Describe your companys return goods policy. 9. Describe your companys proposed implementation plan if you were to enter into an agreement with TRICARE Puget Sound and Alaska, TRICARE Region Southwest and TRICARE Region Pacific. 10. Does your company provide volume discounts? 11. How does your company recommend disposal of your containers? Does this method comply with regulations to minimize impact on the environment? 12. Does your company have a disposal and/or replacement program? If so, please describe your program and its cost. 13.Provide a history of back orders and/or recalls for this product group with dates, duration, cause, how information was communicated t the customer, interim measures and final resolution. 14. Does your company carry latex-free products 15. Do all products meet NIOSH and OSHA standards 16. Do your products use non-toxic, color fast dyes for labeling on containers and labels that are appropriately color coded? 17. Are your products made of plastic shatterproof and puncture resistant material that is durable and non-collapsible, free of heavy metals, and recyclable? 18. Are your products available in clear, translucent, or opaque material? Can the level of the sharps be seen clearly in the container prior to accessing the container? 19. Are your products designed to prevent overfilling? 20. Are your products designed to prevent spillage of the contents if it is tipped over? 21. If your containers have handles are they positioned away from the opening and sturdy enough to handle the weight of a full container? 22. Are the bio-hazardous waste markings clear and visible? 23. Do all the lids have safety features that prevent access to contents and tamper resistant? 24. Are all the lids designed to prevent dislodging? 25. Do all the lids have a counter-balance mechanism for automatic drop? 26. Do all the lids lock securely when full? 27. Are all the lids designed not to allow fingers past drop arm of counterbalanced lid or designed to reduce potential for injuries? Explain 28. Are there a variety of lids available? Provide a list. 29. Will wall mounts be provided at no extra charge? 30. Are your wall mounts designed to mount flush with the wall? 31. Do you offer a full line of wall holders and locking devices? Provide a list. 32. Do you carry floor models large enough to accommodate long disposable instruments such as laparoscopic clip appliers? 33. Do you carry floor models that have a foot operated opening device? Companies interested in doing business with TRICARE Puget Sound and Alaska, TRICARE Region Southwest and TRICARE Region Pacific for Sharps Containers need to reply to this solicitation NO LATER THAN 11 November 2005. All responses will include company name , POC and contact information (suggest more then one POC), your intent to participate in this standardization initiative and a reply to the above questions referenced to by question number.
