- The Carl T. Hayden VA Medical Center is seeking a contractor to provide Ophthalmology Services to eligible beneficiaries. The components of services include: (1) Eye Clinic Services to be performed at the Carl T. Hayden VA Medical Center, and (2) Surgical Services to be performed at the contractor?s facility, to be located in close proximity to the VA Medical Center. The estimated annual requirements are as follows: 1. Eye Clinic ? Contractor, in collaboration with VA, will establish clinic profiles that will ensure timely initial and follow up appointments, based on 19,000 outpatient visits annually. 2. Surgical Services ? Contractor to provide highly qualified, board certified ophthalmologists who can provide a full spectrum of ophthalmology services including corneal, medical and surgical retina, glaucoma, neuro-ophthalmology, oculoplastics and general ophthalmology. Estimated Annual Surgical Cases - 620 consisting of: Cataracts 443, Retinas 69,Corneas 9, Plastics 99. Contract period shall be for a base year beginning January 1, 2006, or date of award, through December 31, 2006 with 1 additional option year, subject to the availability of funds. This acquisition is unrestricted. North American Industry Classification System (NAICS) 621111 and a size standard of $8.5 million is applicable to this acquisition. The solicitation shall be made available on or about October 21, 2005 with a due date of November 22, 2005, by 3:00 p.m. local time.
