- COMBINED SYNOPSIS/SOLICITATION FIBER OPTIC VIDEO SCOPE (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quote (RFQ). Submit written quotes on RFQ reference number F3L3DA5266A300. (III) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-04. (IV) This solicitation is being issued as a small business set-aside; the associated NAICS code is 423690. The size standard is 500 employees. Trying to encourage small business, preferably service disabled veteran owned vendors to reply. (V) This combined synopsis/solicitation is for the following commercial item: Fiber Optic Video Scope. (VI) Description of requirements are as follows: Probe diameter 9mm, length 6m, line of sight forward 0 degrees, field of view 80 degrees, angle of articulation +-120 degrees u/d, +-120 degrees l/r, depth of fucus ?? to infinity. Kit to include 178708 non-conductive articulating videoscope, 10? color monitor display, pressure sensitive control panel, IR and white light sources, digital documentations system, scope mounted bracket for hands free operation, rf receiver/transmitter, ac power supply and dc battery 115v ac charger. (VII) Place of delivery is Peterson Air Force Base, CO 80914; 30 net days after award of contract. (VIII) FAR 52.212-1, Instructions to Offerors ? Commercial Items (Jan 2004), applies to this acquisition. In addition to written quotes, offerors are instructed to provide a capability statement describing qualifications to perform requirements listed at paragraphs V and VI. (IX) FAR 52.212-2, Evaluation -- Commercial Items (Jan 1999), applies to this acquisition. Paragraph A of this clause is completed as follows: 1) Price shall be used to evaluate quotes. (X) The offeror must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (May 2004). A copy of this provision is located at http://farsite.hill.af.mil/ (XI) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (Oct 2003), applies to this acquisition. (XII) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Jun 2004). Additional FAR 52.212-5 clauses cited in the clause applicable to this acquisition are (b) (5i) (14) (15) (16) (17) (18) (26) and (31), (c) (1) and (2). (XIII) The following clauses are also applicable to this acquisition: 52.204-7, Central Contractor Registration; 52.252-2, Clauses Incorporated by Reference; 252.204-7004 Alt A, Required Central Contractor Registration; 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items; including 252.225-7001, Buy American Act and Balance of Payments Program, and 252.232-7003, Electronic Submission of Payment Requests. (XIV) The Government intends to award a Firm Fixed Priced contract resulting from this solicitation. (XV) Quotes are required to be received no later than 12:00 pm MST, 29 September 2005.
