- __________________________________________________________________ AMENDMENT NUMBER THREE, 15 SEP 2005: This amendment changes the fill-in portion of provision 52.212-2 to read: "The evaluation factors for the fill-in portion of this provision are: 1) technical capability of the service offered to meet the Government requirement; 2) past performance; and 3) price." This change has been incorporated into the announcement. This amendment adds a requirement that the potential contractor provide a list of previous Government and/or Commercial Contracts with contract numbers, contract titles, dates of performance, and customer/company name and point of contact information (individual name, phone numbers, email). List shall include only recent, relevant information in the last three years. The format of this list is determined by the potential Contractor but must provide all requested information. If there is no previous performance, the potential Contractor shall state this with the email or fax cover of their quote. This change has been incorporated into the announcement. This amendment clarifies the following in each of the Performance Work Statements: "Contractor is responsible for providing music." This change has been incorporated into the PWSs attached to this announcement. This amendment adds the following to each of the PWSs: "Contractor shall provide a headcount to the fitness center front desk immediately after each class concludes. The headcount shall include (in the following order) Name of Class, Location of Class, Date/Time of Class, Instructor Name, and Headcount. The headcount report shall be in an informal or formal written format of the contractor+s choice." This change has been incorporated into the PWSs attached to this announcement.
