- The VA Medical Center, 4100 West Third Street, Dayton, OH 45428, has a requirement for a Radiofrequency Lesion Generator RFG-3C+ or equal. The North American Industrial Classification Standard is 334510, the size standard is 500 employees. This procurement is unrestricted. The Request for Quotes (RFQ) is due by 3:00 PM, September 22, 2005. All quotes must include the following information: (1) cost of lesion generator with Disctrode capability Radionics Model RFG-3C+ or equivalent (2) warranty one year parts and labor (3) operator?s manuals (two sets) (4) service manual containing parts list, schematics, and wiring diagrams (one each). Interested parties must e-mail or fax quotes to the Contracting Officer listed. Phone requests will not be accepted. Your quote must contain a Point of Contact (POC), address, phone and fax numbers. All bidders are responsible for reviewing the website and for obtaining any and all amendments.
