- STATEMENT OF WORK (SOW) FOR SKATEBOARD CAMP The contractor shall provide non-personal services to include all labor, materials, transportation, including air and ground, lodging and meals, to operate a Skateboard Camp for seven (7) Air Force bases listed below. 1. DESCRIPTION OF CONTRACTOR PROVIDED SERVICES: The contractor shall provide: 1.1. Children ages 9 to 18, an opportunity to learn through experience, the basic skills, newest techniques and safety measures of skateboarding and or roller blading on obstacles at various United States Air Forces in Europe (USAFE) locations. Camps shall be conducted at the following locations during the weeks of 12 Jun 06-31 Aug 06; United Kingdom-Alconbury, Lakenheath, Croughton and Mildenhall; Germany- Ramstein and Vogelweh; and The Azores, Portugal-Lajes. 1.2. Competent instructors with 18 months skateboarding experience, and instruction, and desire and abilities to work with youth. Contractor professionals, with expertise in their field, shall conduct the specialty camps. l.3. Staff that comply with standard youth program?s guidelines in reference to freedom of communicable disease, drug and child abuse and shall adhere to, enforce and encourage all safety and ethic rules of base?s skateboard park. 1.4. Two instructors per class during each session to maintain a 1 to 9 ratio. Class size shall not exceed 27 participants. Camps shall be a week in length and be conducted in 4-hour daily sessions. Camps shall begin on Monday and end on Friday. The goal is to reach maximum participation within safe parameters. 1.5. Lesson plan for a 4-hour daily class for each day of the week to include but not limited to, safety, equipment care, basic fundamentals, and introduction into advance skills. Tailoring the lesson plan to age and capabilities at each specific camp. 1.6. Training session to base staff and volunteers on the basics of operating a skateboard park. To include but not limited to information on: safety and liability; staffing; park design and maintenance. 1.7. Each base with a reference guide including programming operations guide and daily lesson plans. 1.8. Press release: multi-use program release template/overview description, pre-camp promotion poster: designed to appeal and resonate with acting sports youth, easy to use template, size to meet needs, print and post, name tags: up to 27 ?cool? adhesive camper name tags, individual achievement certificates and up to 27 custom logo certificate forms, Skateboard Camp T-Shirts, up to 27 includes creative design, shirt, printing production, and shipping to base in advance of camp for each participant of every camp. 2. SERVICE DELIVERY SUMMARY Performance Objective SOW paragraph Performance Threshold Skateboard camps complying with the SOW requirements 1.1. thru 1.7 and 4.1.1. 95% customer satisfaction at each location at all times and in compliance with the government approved contractor development objectives and program activities 3. GOVERNMENT FURNISHED EQUIPMENT AND SERVICES: None, other than utilities on site for the purpose of performing this SOW, and as indicated below. 3.1. HQ USAFE/SV will provide each individual instructor a Letter of Identification (LOI) for access to military installations and activities. Contractor shall complete LOI personal data 3.2. Travel itineraries shall be coordinated though HQ USAFE/A7SPCY no later than 1 May 06. 3.3. Requirements of the hosting base/services performed by the Government: 3.3.1. Advertisement of the camp 3.3.2. Camp Registration. Youth of deployed parents have priority but are not the only eligible participants. 3.3.3. Create a waiting list in case of participant late cancellation. The goal is to maximize participation for each camp. 3.3.4. The camp is offered free or at very limited cost. A commitment fee sometimes guarantees the participant will show up. 3.3.5. Reserve and pay for lodging rooms for each instructor (2 each) or as needed. If base lodging is not available reserve a near-by contract quarters. 3.3.6 A minimum of one staff member with required background checks shall be present and involved during the entire camp. 3.3.7. Coolers of water and or Gatorade during each session. 3.3.8. A clean and safe skate park at the necessary times open only to the participants in the camp. Youth participant must supply their own equipment to include but not limited to, skateboard, helmet and knee pads. If the Youth Program has this equipment it may be issued to the participants. 3.3.9 Obtain the following information from the contractor: -Instructors Full Name and Social Security for lodging -Approximate arrival time to obtain base entry/access passes - Provide to the contractor: --Directions to the base, lodging and youth center --What is necessary to obtain a visitor?s pass for the base (Driver?s license, passport #) --Time and location for meetings and camp schedules --Any ?unique to your base? instructions --Program?s point of contact and home or cell phone number 3.3.10. Fill out the after action survey completely and fax it back to Commercial # 0049-(0)6302-67-7947 within 7 calendar days of hosting the camp. Attn: Lori Anderson or e-mail to lori.anderson@sembach.af.mil 4. GENERAL INFORMATION: 4.1. PROCESS AND PROCEDURES FOR PERFORMING DUTIES: 4.1.1. Skateboard Camp: The weekly schedule for youth, developmental objectives and program activities shall be delivered on or before 29 Sep 05 to HQ USAFE/SVPCY staff at HQ USAFE/SVPCY, Unit 3050, Box 160, APO AE 09136, ATTN: Ms. Lori Anderson or Ms Jan Seachris. Based on the plan developed, execution of the skateboard camp will be on above seven (7) selected bases FROM 12 Jun 06 thru 31 Aug 06. 4.1.2. Logistical Requirements : All logistics shall be coordinated with host site project officers once Skateboard Camp Officials and HQ USAFE/SVPCY have determined the summer 06 schedule based on the curriculum provided by the contractor NLT 29 Sep 05. This includes 7 specific weekly scheduled camp sessions with times and locations as listed above.
