- The VA Great Lakes Acquisition Center intends to negotiate a sole source contract with Gordon N. Stowe & Associates, Inc. to provide (1) GSI Audera 2-Channel Desk Top EP / ASSR / DPOAE System for the VAMC Iron Mountain, 325 EAST ?H? STREET, IRON MOUTAIN, MI 49801. To the best of our knowledge, the GSI Audera 2-Channel Desk Top EP / ASSR / DPOAE System is the only equipment which will meet the needs of the facility and offers both a Polar Plot of the Phase Angels of the collected response signals for DPOAE and averaged auditory neural latency response signals for ASSR and EP assessment. The equipment is compatible to the existing equipment at this facility. This procurement is being conducted in accordance with FAR 6.302 - only one responsible source and no other supplies or services will satisfy agency requirements. Contract type is firm-fixed price and the NAICS Code is 334510. Equipment required for this location must include the following characteristics: equipment will be able to assess both cochlear and retro-cochlear with AEP testing capabilities such as electrocochleography, auditory brainstem responses, auditory middle latency, long latency, cortical responses, vestibular evoked myogenic potentials. This Notice of Intent is not a request for competitive proposals. No solicitations documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service, to the Contracting Officer within (10) calendar days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information must be sent to the Contracting Officer, VAMC GLAC, 5000 W. National Ave Bldg. 5, Milwaukee WI 53295 or via fax to (414) 902-5440. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with Gordon N. Stowe & Associates, Inc.
