Description

The VA Medical Center, 4646 John R Street, Detroit, MI 48201 has a requirement for annual preventive maintenance of 10 government owned Bear 1000 ventilators to include inspection. Service and repairs parts will be billed separately. The contractor shall perform annual maintenance of government owned Bear 1000 ventilators. This contract will be for a base period of one year, beginning September 30, 2005 through September 29, 2010, with four pre-priced option years. Contractor must be an authorized, licensed factory trained rep. Contract shall include all applicable software revisions within the original purchased version of operating software at no additional charge. Contractor shall provide preventive maintenance documentation for hospital accreditation inspections. Preventive Maintenance contract shall provide for all labor and travel charges plus PM parts required to complete the PM. Contractor shall include inspection and service of ventilators on an annual basis at the prices quoted in the pricing schedule. Contractor shall provide all tools, labor, transportation, supervision and parts necessary to provide full maintenance including preventive maintenance.
