- This is a combined synopsis/solicitation for commercial items not prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number F2P4MD5165A300 is issued as a Request for Quote (RFQ) and details can be obtained by contacting Hazel Long, Contracting Specialist at UK 1638 52 2354 or e-mailing hazel.long@lakenheath.af.mil. The U.S. Air Force, 48 Contracting Squadron intends to award a firm fixed price contract for Period of Performance 1 October 2005 to 30 September 2006 for the Licence Renewal of a system to help Emergency Physicians and Primary Care Physicians records at the hospital, RAF Lakenheath, England. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. Award will be evaluated on technical specifications and total price. A completed copy of FAR 52.212-3 Offeror Representation and Certification ? Commercial Items will be required and interested contractors should be registered with CCR at www.ccr.gov.
