Description
This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format in FAR, Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation issued. STATEMENT OF WORK: Quotes are being requested and a written solicitation will not be issued. The solicitation number is 247-0056-06 and is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-27. This is a 100% set-aside for small business procurement. NAICS is 446110.The small business size is 500 employees. The Department of Veterans Affairs Birmingham, AL requires the following; (1).Contractor shall provide all labor, supervision and transportation necessary to deliver doses of Cardiolite/Sestamibi to the Birmingham VA Nuclear Medicine. (VA will furnish vials of cardiolite 6 doses per vial for compounding). (2) ALL RADIOPHARMACEUTICAL orders shall be within +/-10 percent (%) of the doctor's order allowing only for correcting calibration date and time. All radiopharmaceuticals shall be in total compliance with State of Alabama and/or as well met all Federal Regulations regarding radioactive materials. (3) ALL SHIPPING CONTAINER shall be free of radioactive contamination both inside and outside; shall met and be in compliance with all state and Federal regulating agencies, Department of Transportation (DOT) and Nuclear Regulatory Commission (NRC) as required by Law. (4) CONTRACTOR shall supply with each shipment/delivery/order a system for spent (used) syringes that meet OSHA's Biohazard policy and is needle less. (5) EMERGENCY DELIVERY: In case of emergency, the contractor shall be required to make emergency deliveries within one [1] hour, excluding week-ends and Federal Holidays as requested by Nuclear Medicine Personnel and/or physicians. (6) LICENSE: A copy of the Department of Veterans Affairs Medical Center's Nuclear Regulatory Commission license will be furnished to the contractor upon award of contract. (7) ADDITION/REDUCTION/CANCELLATION: VA shall have the option to add, reduce and/or cancel an order by giving the contractor at least one hour notice. There will be no additional charge for anything, prep charges or delivery charges. THE SCHEDULE OF PRICES FOR THE BASE YEAR AND TWO (1) Year OPTION beginning October 1, 2005 shall be listed and priced as follows: Estimated Does 2600, "compound fee per dose" $________. $__________ Total. Delivery FOB Destination to Birmingham VA Nuclear Medicine, 2nd floor, 700 south 19th Street, Birmingham, AL. 35233. Submit offers on Company Letterhead stationary in accordance with FAR 52.212-1 Instructions to Offerors commercial Items. Acceptance time will be 60 days from offer due date. The following provision apply to this solicitation: 52.212-1, 52.212-3 Representations and Certifications-Commercial Items; 52.212-4 Contract Terms and Conditions; 52.204-7 Central Contractor Registration; 52.212-1 Solicitation Provisions Incorporated by Reference; 852.270-4 Commercial Advertising; 852.271-70 Nondiscrimination in Services Provided Beneficiaries; 52.252-2 Clauses incorporated by reference including Clauses incorporated by reference under Section (b) 52.232-18 Availability of Funds; 52.232-38 Submission of Electronic Funds Transfer Information with Offer; 52.233-2 Service of Protest; 52.233-4 Applicable Law for Breach of Contract Claim. Interested parties are required to submit prices, clear and convincing data substantiating their capabilities and qualifications to satisfy this requirement. The Government contemplates award of a firm fixed price single award contract resulting from this solicitation. All offerors responding to this announcement shall submit an offer along with the information required in 52.212-1 and a completed copy of provision 52.212-3, to Contracting Officer, Central Alabama Veterans Health Care System, East Campus 2400 Hospital Road, Tuskegee, AL 36083 by 4:30 PM local time, 09/16/2005.
