- Amendment 0001--The following information is added to this combined synopsis/solicitation: On the positive pressure ventilator we need a 110v AC electric. The 10'x10' tent we need a nonpermeable material with a separate front and rear means of entrance and egress for decon/changing area. The self supporting tank, it is for containing and transporting runoff from decon operations I would like a 500 gallon capacity preferably made of poly-urethane or like material. Quotes should be submitted via email by 11:59 PM EST on 18 September 2005. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The following items are required. 1. Positive/Negative Pressure Ventilator with explosion proof motor. Quantity required: 4 2. Hazmat Response Tool Kit. The kit should include: Shoe handle brush 9" crate opener 16 oz. claw hammer with fiberglass handle Common knife 53/4" blade, 73/4" OAL Deck Scraper, 11/2" blade x 15" long Spray booth scraper, 3" blade x 91/4" long Standard screwdriver--5/16" tip, 6" blade 12" groove joint pliers #3 phillips screwdriver 12" tin snips 8" adjustable wrench 12" adjustable wrench 14" pipe wrench (aluminum) 12" bung wrench (fits 3/4" x 2") 7" long nose pliers with cutters 8" combination pliers. Quantity required: 2 3. Emergency Kit 'A' for 100-150 LB Chlorine Cylinders. Quantity required: 2 4. Hazmat Patch & Plug Response Kit with non-sparking tools. The kit should include sheet metal screws with neoprene compression seals. Tubeless tire repair kit. OSHA-approved 200-foot roll of barricade tape. One 8" x 12" piece of solid neoprene and one 22" piece of foam material. Lead wool epoxy putty that hardens like steel, hot or cold hose repair tape, basic tool set for patching, twin ?T? stainless plate with foam, an 8" x 12" stainless backed neoprene patch-hardwood cribbing, two 22' nylon straps and buckles. Two ?T? bolt crack patches for awkward-shaped breaks. Six surface plugs for curved surfaces, wedged wooden plugs covered with felt and 2 ball plugs. Carrying case included with the purchase of this Hazmat kit. Quantity required: 2. 5. Hazmat Patch & Plug Response Kit with Non-sparking tools. The kit should include ten pipe bandages for covering pipe sizes from 1/2" to 4", soft and hard neoprene liner material. A carrying case should be included with the purchase of this Hazmat kit. Quantity required: 2. 6. Rapid tent 10 x 10 size with a rear door. Quantity required: 2. 7. Self supporting frameless portable water tank. Quantity required 2. 8. Extrication Gloves.
