- This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.603 and supplemented with information included in this notice. This announcement is the only solicitation. A written solicitation will not be issued. Quotes are being requested. This requirement, number FA4427-05-Q-0052, is issued as a Request for Quotation (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-05 and Defense DCN 20050901. This solicitation/acquisition will be 100% small business set-aside. The Federal Supply Class (FSC) is Q515. The Standard Industrial Classification (SIC) is 8071. The North American Industry Classification System (NAICS) is 621511. The associated small business size standard is $11,500,000 annual revenue. This RFQ consists of the following item. Item 0001: Services non-personal: contractor to provide emergency renal biopsy testing and test interpretation to include light microscopy (LM), immunofluorescence (IF), and electron microscopy (EM) for the period 01 Oct 05 through 30 Sep 06 in accordance with the Performance Work Statement. It is estimated that there will be sixteen (16) tests per year. The contractor shall provide all personnel, materials, facilities, supplies, and any other services and items necessary to perform this special testing for renal biopsies, as well as interpretation of the testing results, when required by the Military Treatment Facility (MTF) at David Grant Medical Center, Travis AFB, CA. The contractor shall comply with the Joint Commission Accreditation Healthcare Organization (JCAHO), Food and Drug Administration (FDA), Privacy Act of 1974, Health Insurance Portability and Accountability Act (HIPAA), and all Federal, State, and local laws applicable to performance of work. The following requirements must be met: (a) renal biopsy testing and interpretation services 365 days per year including Federal holidays. (b) renal biopsy tests must include but are not limited to: Light Microscopy (including PAS, Jones Silver, Gomori Trichrome) CPT codes 88305 and 88313; Immunofluorescence (including IgG, IgM, IgA, C3, C4, C1Q, Albumin, Fibrinogen, Kappa and Lambda Light Chains) CPT code 88346; Electron Microscopy CPT code 88348; Consultation on Slides CPT codes 86021 and 86038; Additional Special Stains (AFB, Fungus) CPT code 88312; Additional Special Stains (Amyloid) CPT code 88313; Additional Immunofluorescence Stain CPT code 88346. (c) 24 hour turnaround time for renal biopsy testing, which is to include interpretation of test results. (d) online reporting of test results at no additional charge, a fax copy of the results as soon as they are available, and a paper copy of the results via regular mail for documentation purposes. (e) new medical testing shall not be introduced without prior recommendation and approval of the medical facility Commander and the Contracting Officer. (f) incorporate the MTF?s Central Operations patient numbering sequence on the report. (g) single-site (one lab) specimen testing for easy specimen tracking. (h) one month advance notification of test changes, additions, and deletions. (i) provide the scheduled days of testing for the different testing procedures with turnaround times. (j) provide monthly electronic invoices with paper invoices mailed for documentation purposes. (k) provide a client service representative Point of Contact (POC) that can be reached 24 hours a day, 7 days a week. Furthermore, these 3 service performance objectives must meet 100% compliance: (a) single-site (one lab) specimen testing 24 hours a day, 7 days a week. (b) provide preliminary test results within 24 hours of receipt of specimen. (c) properly maintain and dispose of records. The following provisions and/or clauses apply to this acquisition.
