Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Instructions To Offerors Proposal Submission 1. Offers will be received no later than Friday 16 September by close of business e-mail them to joanne.botwinick @us.army.mil. The Period of Performance is 30 September thru 9 December 2005 with 4 Option year s and subject to the availability of funds. 2. The following is to be submitted as part of your offer: a. Resume: A one page document , which identifies 1 year of experience operating a program of community Information and Referral assistance within the Army Community Service. b. Three references: References can be personal or professional family members are excluded. reference information shall include name, address, telephone number, and an indication that the reference is personal or professional. Th e Government may contact these references with regard to past performance. c. The Contractor must be New Jersey based and within a fifty mile radius of Fort Monmouth. d. The Contractor may be required to submit to an interview. PERFORMANCE WORK STATEMENT ARMY COMMUNITY SERVICE INFORMATION & REFERRAL PROGRAM MANAGER C.1. GENERAL The purpose of the Army Community Service Information & Referral Program is to provide information & referral assistance located within the Army Community Service (ACS) to the Fort Monmouth community. ACS is a community oriented Social Service Program to assist eligible clients as set forth in Army Regulation AR 608-1: The Army Community Service Program is hereafter referred to as AR 608-1. C.1.1. SCOPE OF WORK The Community Information & Referral Program Contractor (hereafter referred to as Contractor) shall implement the Community Information & Referral Program to assist eligible clients as set forth in AR 608-1 and act as the primary point of contact for the o rchestration and execution of all Community Information & Referral Program efforts whether or not he/she is actually involved in the direct service delivery of each initiative. C.1.1.1. The Contractor shall provide support services to ACS through the ACS Information & Referral Program for eligible clients as set forth in AR 608-1. The contractor shall provide the professional and administrative support and personnel to perform the servic e set forth in this Performance of Work Statement (PWS). This PWS consists of the following principle features: C.1.1.2 The contractor shall provide a program manager for forty (40) hours each week, less 10 approved U.S. holidays. The contractor shall provide a qualified and trained substitute for periods of time the designated program manager is unavailable to provide services in excess of three (3) working days in a two-week period. The contractor will identify all absences (except for emergencies) during the duration of the contract within two (2) weeks of executing the contract and will provide a written plan to the COR, to cover every aspect of the contract during the designated pr ogram managers absence. The contractor shall ensure that, in the event the program manager will arrive late to the work site, he/she must contact the COR within 30 minutes of the duty day. The contractor shall be responsive, within 24 hours, to telephone calls or e-mails from the COR or contracting office regarding coordination of the contract. C.1.1.3. Services shall be provided during operational hours from 0800-1645, Monday through Friday. C.1.1.4. Primary work site will be at Army Community Service, Building 812, Murphy Drive, Fort Monmouth, NJ. C.1.2. PERSONNEL. C.1.2.1. The contractor shall have the following documented, experience, knowledge, skills, and abilities: C.1.2.1. 1. Experience in providing services to military families. C.1.2.1.2. Basic knowledge of services offered by all other ACS programs which include: Financial Readiness, Family Advocacy Program (FAP), Employment Readiness, Information & Referral, (I&R), Army Volunteer Corps, Relocation Readiness, Deployment or Mobilization and Stability Support Operations Readiness, and Outreach Services. C.1.2.1.3. The ability to communicate effectively; both in writing and verbally. C.1.2.1.4. Skills in using a personal computer, the internet, e-mailing and the following programs: Microsoft Word, Excel, and Publishing and Powerpoint. C.1.2.1.5. GOVERNMENT FURNISHED PROPERTY (GFP) C.3.1. The Government will provide to the Contractor for use in connection with this contract the following property: C.3.1.1. Building - sufficient desk space for one program manager. C.3.1.2. One class A telephone, to be shared with other staff. C.3.1.3. Heat, light, and other essential utilities. C.3.1.4. One computer for program implementation, as well as other equipment and supplies.. C.3.1.5. Standard U.S. Army and Government forms and regulations, as required. C.3.1.6. Office furniture, to include desk, chair, filing cabinet, bookcase, and storage space. C-4 CONTRACTOR FURNISHED PROPERTY C.4.1. The contractor shall not be required to furnish any property or materials relevant to this contract, but shall be responsible for receiving, caring and accounting for and returning any government furnished prop erty. The contractor shall familiarize him/herself with the proper care and use of government supplied property. The operation of any necessary equipment, to include training aides within the services to be delivered hereunder shall be at no additional e xpense to the government over and above the contract price. C.5. SPECIFIC TASKS C.5.1. The program manager shall provide assistance to ACS customers. Customers may present as a walk-in, with an appointment, or by telephone or e-mail. The program manager will assess, through a short interview with the client, the customers request and type of assistance required. In more complex situations, the program manager will refer ACS customers to specific ACS program managers, i.e., Financial Readiness, Employment Readiness, Family Advocacy, etc. The program manager will maintain a current resource file of military and civilian information commonly requested by ACS customers. C 5.1.1. The program manager shall have responsibility for administering and maintaining the Utility Deposit Waiver Program UDWP which is an agreement between Bell Atlantic, Jersey Central Power & Light, and New Jersey Natural Gas and the Commander, Fort Monmouth t o assist military personnel assigned to Fort Monmouth. C.5.2. The program manager shall act as a Unit Service Coordinator to individual units as determined by COR. C.5.2.1. The program manager shall receive cross training in all other ACS program areas as determined by COR. C.5.2.2. The program manager shall provide program specific cross training to ACS contractors, volunteers and government employees as determined by COR. C.5.3. The program manager shall have responsibility for administering and maintaining the Lending Closet, which makes available a variety of basic housekeeping items for temporary loan to eligible clients. C.5.3.1. The program manager shall provide information ranging from limited factual response to more detailed factual information about community service systems, agency policies, and application procedures. C.5.4. Record keeping The program manager shall gather and maintain a variety of data as required by governing program guidance to support management information needs. C.5.4.1. The program manager shall gather, maintain, and analyze data for planning and reporting purposes on a monthly basis. C.5.4.1.1. The program manager shall provide a written report of data on a monthly basis and input data into the ACS Management Report or other reporting mechanisms. C.5.4.1.3. The program manager shall provide a written report of data on a yearly basis. C.5.4.2. The program manager shall develop, implement and maintain an SOP for the Lending Closet. C.5.4.3. The program manager shall develop, implement and maintain an SOP for the Utility Deposit Waiver Program (UDWP). C.5.4.4. The program manager shall maintain a weekly record of significant activities, as defined by COR. C.5.4.5. The program manager shall record daily hours worked in implementation of contract. C.5.4.6 The program manager will publish a monthly an ACS newsletter by the 20th day of the month. C.5.4.7 The program manager will provide customers with comment cards to ascertain their satisfaction with services received. C.5.4.8 The program manager will provide minutes of weekly ACS staff meeting to the COR within 3 business days. C.5.4.9 The program manager shall maintain a neat and professional workspace at the ACS front desk and waiting area. C.5.4.10 The program manager shall assist soldier and families who present to ACS during in and out processing from Fort Monmouth. C.5.4.11 The program manager shall maintain a neat, well-groomed professional manner and maintain decorum and language conducive a professional setting. C.6. APPLICABLE DOCUMENTS C.6.1. FM-R 690-090.2 Clearing of Civilian Personnel C.7. DELIVERABLES C.7.1. The program manager shall provide an individual participation file for each participant in the UDWP within 2 days of enrollment. C.7.2. The program manager shall provide a record of participation for each individual using the lending closet immediately upon use. C.7.3. The program manager shall provide the COR with a list of supplies and materials required for lending closet in January. C.7.4. The program manager shall provide an invoice of hours provided in implementation of contractor to COR by the 1st of each month. C.7.5. The program manager shall provide a Memorandum of Record within 2 working days to report an incident. C.7.6. The program manager shall provide a completed DA FORM 5900-R per AR 608-1 to the COR by the 15th of next month. C.7.7. The program manager shall provide a weekly Significant Activities report by 12:00 p.m. on the following Monday. C.8. Transportation The contractor will supply his or her own transportation to perform the contract. Subject to the availability and at the authority of the COR, the contractor may use government vehicles, provided a government motor vehicle license is obtained. Application and testing to obtain this license will be during the contract hours. C.9 Quality Control The contractor shall establish a quality control program to ensure that the requirements of the contract are accomplished as specified. Initial plan shall be presented to the COR within 30 days of contract award for approval. C.10 Confidentiality The contractor shall maintain proper confidentiality regarding personal information of clients and volunteers, to include social security numbers, telephone numbers, and interviewing information. C.11 Physical Security The contractor shall adhere to established physical security and internal control procedures as directed by AR 215-1. The contractor shall be responsible for securing any building or work area of which he/she is the last user of the day. Locking of external doors and windows is necessary when leaving the facility. The contractor shall have keys issued to them through the ACS Office Key Control Officer for Building 812. The contractor will report any occurrences of lost keys to the COR and ACS Key Control Officer within 24 hours of the discovery of the loss. The contractor will provide to the COR a written report by 12:00 noon the next day providing all details of lost k ey occurrences. The contractor shall be responsible for any keys issued to him/her and for all costs incurred, if lost, such as replacement of locks. All keys issued shall be returned to the ACS Key Control Manager upon expiration or termination of the contract. C.12 Security The contractor shall take, pass and print the certificate for the computer security test within seven days of award of contract. A copy of the certificate shall be provided to the COR. M.1. Evaluation of Bids/Offers APR 1984 A bidder/offeror must quote on all items in this solicitation to be eligible for award. All items will be awarded only as a unit. Evaluation of bids/offers will be based, among other factors, upon the total price quoted for all items. M.1.1. A bidder/offeror must provide a resume or documentation of eligibility specific to Performance Work Statement PWS SECTION C paragraph 1.2. M.2. Evaluation of Options JUL 1990 Except when it is determined in accordance with FAR 17.206 b not to be in the Governments best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evalu ation of options will not obligate the Government to exercise the options. End of provision
