- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; propo sals are being requested and a written solicitation will not be issued. Solicitation W81K00-05-T-0844 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acqui sition Circular 2005-05. This acquisition is solicited on an unrestricted (full and open) basis under NAICS code 334510 and the size standard is 500 employees. The requirements of this solicitation are: CLIN 0001, 1 each GYNECARE THERMACHOICE III UTERIN E BALLOON THERAPY SYSTEM TO INCLUDE PART NOS. 00825, 01105, 04995, XWE00595, TC003, and CLIN 0002, 1 each GYNECARE VERISTAT FLUID MANAGEMENT SYSTEM TO INCLUDE PART NOS. FS2500, TU0735, FS0950, FS0980, FS0920, and CLIN 0003, 1 each GYNECARE VERSAPOINT SYSTE M TO INCLUDE PART NOS. 00482, 04995, 00481, 00480, 01936, 01939, 01931, 01735, 01934, 01950, 01985, 00468, 00467, XWE00482, 01932, 01933. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand Name or Equal applies to this acquisition. Award will be on an all or none basis. CLIN 0001 salient characteristics are: System controller, umbilical cable, power cord, 1 year extended warranty with loaner, disposable silicone balloon catheters, minimally invasive, thermally 8 minute therapy cycle. CLIN 0002 s alient characteristics are: Fluid management system, inflow tubing pump cartridges, 4 spike, with pressure sensor (6 ea), single use receptacle liners (6 ea), single use link tubing for tandem receptacle setup (24 ea), single use vacuum filter tubing sets (24 ea). CLIN 0003 salient characteristics are: Generator, power cord, foot pedal, hand piece, Hysteroscope, outer sheath-rotatable, working element, Hysteroscopic bridge, std obturator, electrode 0 degree, angled loop electrode, spring tip electrode, twi zzle tip electrode, 1 year maintenance agreement, inner sheath, outer sheath. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by access ing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Irwin Army Community Hospital, 600 Caisson Hill Rd, Property Management Section Attn: Janet Lowrey, Fort Riley, Kansas 66442-5037. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All Offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program . The CCR can be accessed via the Internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Technical Capab ility (compliance with the requirements of FAR Clause 52.211-6 if vendor offers an Equal Product) and Price, with Technical Capability being significantly more important than Price.
