COMBINED SYNOPSIS 100% SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESS SET-ASIDE REMOVE, DISPOSE, PROVIDE & INSTALL CARPET 4 TS, BLDG 4517, 2ND FLOOR Solicitation No: FA4809-05-Q-5017 This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6 and FAR FAC 2005-05. This announcement constitutes the only solicitation, and is issued as a Request for Quotation. A WRITTEN SOLICITATION WILL NOT BE ISSUED. This is a 100% Service-Disabled Veteran-Owned Small Business Set-Aside. This is a firm-fixed price procurement and will be procured using commercial procedures. Solicitation Number is FA4809-05-Q-5017. The NAICS code 238330 applies to this solicitation. The size standard is $12,000,000.00. The item description is as follows: Remove, Dispose, Provide and Install Carpet. Remove old carpet and dispose of 2ND Floor, BLDG 4517, 4 TS, estimated Square Yardage is 960 SQ YDS (Does not include Rooms 209 & 212B). Vendor will determine actual square yardage required through coordination with 4 TS Point of Contact. Vendor will provide and install carpet tiles (24? x 24?), J&J Commercial, STYLE: IMPULSE, COLOR: BEND A RULE # 8401 ?Or Equal?. Vendor will be responsible for moving furniture and replacing replacing as required (does not include disconnecting or reconnecting electronic devices). 4 TS personnel will reove personal items from furniture prior to movement of furniture. Vendor will be responsible for any furniture damage associated with movement of furniture to facilitate carpet installation. Vendor will also remove, replace, dispose, provide and install floor baseboard material (4? base, vinyl), Color: Dark Brown. CLIN 0001: Remove, Dispose, Provide and Install Carpet. Remove old carpet and dispose of 2ND Floor, BLDG 4517, 4 TS, estimated Square Yardage is 960 SQ YDS (Does not include Rooms 209 & 212B). The following clauses and provisions apply to this acquisition: FAR 52.212-1 Instructions to Offerors-Commercial Items; The RFQ should be submitted and shall contain the following information: 1) RFQ number, 2) time specified for receipt of offers, 3) name, address, and telephone number of offeror, 4) terms of any express warranty, 5) price and any discount terms, 6) a completed copy of the representations and certifications at FAR 52.212-3 Alt 1 Offeror Representations and Certifications-Commercial Items (see FAR 52.212-3(j) for those representations and certifications that the offeror shall complete electronically); Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration, (IAW FAR 4.1201 prospective contractors must be registered in the Online Representative & Certification Application (ORCA) at http://orca.bpn.gov ) 7) Period for acceptance of offers - The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation, 8) Late offers - Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is ?late? and will not be considered.
