- This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in the notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The procurement is being issued as a request for quotation. All previous mention to sole sourcing has been recinded. Submit offers on Solicitation Number: 2005-Q-02298. The solicitation is being issued as a request for quotation. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Action Circular 2005-04, dated June 8, 2005. This acquisition will be processed under Simplified Acquisition Procedures (SAP), Acquisitions of Commercial Items and is not a Small Business Set-Aside. The North American Classification System (NAICS) code for this procurement is 541720. The Center for Disease Control and Prevention has a requirement for the following services: Prepare and test pilot teaching modules on a) US Population Structure: An Agin America b) Life Expectancy and Mortality and c) Disability Trends among the US aged Population. CLIN 0001,Prepare revisions of teaching modules and plan for pilot testing in undergraduate courses at 2 locations; CLIN 0002,Report on workshops held at university locations; CLIN 0003, Report on workshops held at 3 other university locations; CLIN 0004 Prepare and deliver final version of undergraduate teaching modules suitable for distribution. The quotation shall include the itemized price, prompt payment terms, F.O.B. (Destination or Origin), Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNS), and descriptive literature of the products quoted. Quotations must reference the Solicitation Number 2005-M-02298. The Delivery Location is Acquistion and Assistance Field Branch 3311 Toledo Road, Hyattsvill, MD 20782. Quotation will be due no later than 4pm on August 31, 2005. Written quotations must be submitted via e-mail to ireid@cdc.gov, Telephone number 770-488-2452, Fax number 770-488-2670. Quotations must be signed by an authorized company representative. By submission of an offer, the offeror acknowledges the requirements that are prospective awardees shall have posted their FAR 52.212-3 Offerors Representations and Certifications Commercial Items at the website: http://orca/bpn.gov. Award will be made to the technically acceptable lowest price. All vendors must be registered in the Central Contractor Registry (CCR) prior to award of a Federal Contract. Vendors may be register at www.ccr.gov . Copies of the above reference clauses are available at www.acqnet.gov. All responsible sources that can meet the above requirements may submit a quote, which will be considered by the agency.
