Description
This amendment incorporates referenced Statement of Work: Statement of Work Preventive Health and Wellness Services American Forces Information Service 1.0. SCOPE: This Statement of Work (SOW) covers a preventive health and wellness program to promote physical and mental health fitness for employees of the American Forces Information Service (AFIS) located at 601 N. Fairfax St., Alexandria VA 22314. These services include membership benefits and responsibilities, wellness program participation and reporting requirements. 2.0. Standards: 2.1 Contractor shall conform to all Federal, state and local health and safety regulations. 2.2 Contractor shall maintain all required commercial licenses and permits in accordance with customary commercial business practices. 3.0. REQUIREMENT: 3.1. Full Fitness Memberships: The Director has authorized participation in the Health Promotion Program by unit civilian and military personnel during their lunch hour. To comply with this time constraint and to facilitate a thirty-minute training regiment, not to include changing and shower time, the facility should be within a 10- minute walking distance from the office location. Vendor shall provide all facilities, equipment, locker areas, showers and full membership privileges to government employees of AFIS. The membership fee will be paid monthly via the government purchase card for all active members and prorated at a monthly rate of one- twelfth the annual fee for partial year memberships. An AFIS Authorization Form is provided to each employee providing the amount paid by the government, as well as the employee portion (Attachment 1). Under no circumstances will an AFIS employee be provided a membership without the appropriate authorization from AFIS. 3.1.1. Co-Pay Membership: This membership is for all civilian employees and those military employees of the rank of O-3 and above. The member?s portion currently is $22.00 monthly. The government will fund all additional membership cost. If the co-pay amount changes, AFIS will notify the vendor using the AFIS Authorization Form. 3.1.2. Fully Subsidized Membership: Membership for military members below the grade of O-3 will be paid in full by AFIS. 3.1.3. Partial Year Memberships: This service is provided at the beginning of the contract to employees that are expected to depart during the contract period. Payment is provided at a prorated share of employee and government portion of annual membership dues. 3.1.4. Prorated memberships: Shall be provided for employees joining after the beginning of the contract period. Payment is provided at a prorated share of the employee and government portion of the annual membership dues. 3.2. Health Assessment and Exercise Prescription: Assessment includes a sub maximal stress analysis, body composition, and strength and flexibility analysis. This service is required by vendor to determine fitness level and ensure proper use of equipment. The cost of this assessment will be funded by AFIS. Personnel renewing memberships do not require health assessments, but will be provided updates to their exercise prescriptions at no additional charge. 3.3. Guest Membership Passes: To be used by AFIS sponsored military and civilian personnel that are on government travel orders visiting the AFIS facility. Permanent AFIS employees are not authorized use of a guest pass unless their membership is covered in full by the government and it is more advantageous to provide guest passes instead of a short-term membership. 3.4. Sale of Memberships: Any member shall be able to sell or transfer their membership to other AFIS personnel within the contract period with prior approval from AFIS Operations Manager. The administrative fee associated with the transfer will be paid by AFIS. The new member will be authorized a Health Assessment and the fee shall be paid by AFIS. 3.5. Probationary Memberships: Probationary memberships are required for employees not meeting the government?s minimum usage requirements of 8 times per month and requesting to renew their membership. A probationary membership will be for a three- month period starting at the beginning of the subsequent contract period to the substandard usage period. AFIS will pay a prorated portion for the probationary period. The employee will also pay a prorated portion for their share of the membership costs, if applicable. Under no circumstances will an AFIS employee on probation be provided an annual membership without prior authorization from AFIS Operations Manager. 3.6. Aerobic Room Rental: Room rental is required 4 times each year for the official military physical fitness test. Room rental is for one hour and includes use of scales and locker/shower areas. Some of the personnel using the facility for the testing may not be members of the facility. One-month advance notice for scheduling and coordination with vendor point of contact will be provided by AFIS. 3.7. Reporting Requirements: The vendor shall provide a usage report on a minimum of a quarterly basis, with the quarters beginning in October of each year. Quarterly reports are due within 5 days of the end of each quarter. Usage reports may also be required on a weekly, or monthly basis. (See Attachment 2 for format). Usage reports are an integral part of the Fitness Program because AFIS requires personnel to use the facility a minimum of two times per week, and an average 8 times per month. In the event the facility monitoring equipment is inoperable, vendor personnel shall manually track usage of AFIS employees and later update the system. Accuracy is imperative, as it is the basis for approval to join the facility. 3.8. Wellness Program: All AFIS personnel are authorized one Nutritional Analysis and Lifestyle Consultation per year regardless of membership with health facility. 3.8.1. Nutritional Analysis and Lifestyle Consultation: The health facility shall provide individual nutritional analysis for any AFIS personnel presenting an authorization form. The analysis shall consist of a questionnaire- based compilation of nutritional intake for each individual for a one- week period. The consultation shall include the results of the health assessment and the prescription of exercise activities and dietary programs. 3.9. Authorization Forms: Any of the above services requested by AFIS will be accompanied by an AFIS Authorization Form, (Attachment 1) signed by AFIS Operations Manager, Chief of Logistics or Chief of Administration authorizing the services for payment and providing a fee charged to AFIS, as well as the member. No services or memberships will be provided by facility without an AFIS Authorization Form. 3.10. Payment Invoices: Payment shall be made with the government purchase card. 3.10.1. Membership: Annual membership fees will be invoiced and paid monthly as services are received. If an invoice is greater than $25,000 payment shall be made by electronic funds transfer from the Defense Finance and Accounting Service (DFAS). All invoices less than $25,000 shall be paid using the government purchase card. 3.10.2. Services: All services beyond the membership shall be invoiced at the time the services are rendered. 3.10.3. Co-Pay: At the beginning of the contract period, the employee?s share of membership dues may be paid in full at the beginning of the contract period, or over a three-month period, charged to a Visa/Master Card or electronic funds transfer by using personal check. The co-pay portion of prorated memberships will be paid in full by the employee at contract initiation. 3.10.4. Other Requirements: Vendor shall provide the members names on each invoice for services billed. 3.11. Points of Contact: The contractor shall provide a single point of contact for all membership orders, service request, and billing questions during regular business hours.
