Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The RFQ for this synopsis/solicitation is 667-05-4-132-0150 and is subject to the availability of funds. Currently, funds are not available and the Government shall not be under any obligation to purchase. The North American Industry Classification System (NAICS) code is 423450 and size standard is 500 employees. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This acquisition is unrestricted. This acquisition is for the purchase, delivery and installation of three non-mydriatic retinal camera systems to be distributed in three different locations. The locations are at the Department of Veterans Affairs, Community Base Outpatient Clinics (CBOC?s) located in Longview, TX, Monroe, LA and Texarkana, AR. The offeror shall provide pricing and warranty information on the following equipment: Topcon (Brand Name) TRC-NW200 Camera with DVS System, AIT-15 Table and LCD Monitor. System is to include Lombart (Brand Name) DVS Network Station Computer. If a different system is being bid on other than that described, the offeror shall provide sufficient information with their quote as to compare the systems. Delivery is desired between October 3, 2005 and October 23, 2005. Inspection and acceptance will be at each of the above listed CBOC?s. The provision at FAR 52.212-1, Instructions of Offeror?Commercial Items (Jan 2005) applies to this acquisition. There is an addenda to these provisions. Add the following provisions: FAR 52.252-1, Solicitation Provisions Incorporated by Reference (Feb 1998). This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provision may include blocks that must be completed by the offeror and submitted with its quotation or offer. In lieu of submitting the full text of these provisions, the offeror may identify the provision by paragraph identifier and provide the appropriate information with its quotation or offer. Additional contract requirements: The vendor responding to this acquisition shall provide this office with the following information: a cost per item on company letterhead (equipment and consumables supplies), stating unit price, warranty, FOB, prompt payment terms, correct remittance address, and a completed copy of FAR 52.212-3 (Mar 2005), Offeror Representations and Certifications?Commercial Items. If offering an equal product, the vendor must submit documentation of the product with the quote. Quotes must be in writing and accompanied by a completed Representations and Certifications (FAR 52.212-3) dated Mar 2005 or they will be rejected. Facsimile quotes will be accepted. Oral quotes will not be accepted. Award will be made to the offeror whose quote conforms to the requirements of this solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered.
