Description

This is a combined synopsis/solicitation issued for commercial items. The VA Network 22 Logistics Office, Long Beach, California 90822 intends to negotiate a commercial (FAR Part 12) sole source contract with Endocardial Solutions, 1350 Energy Lane, Suite 110 Saint Paul, MN 55108-5254 under the authority of FAR 6.302-1. A justification for other than full and open competition has been prepared in support of this acquisition. The Request for Proposal #RFQ 600-270-05BV entitled "Ensite 3000 System, with supplies is hereby issued for Commercial items and is prepared in accordance with the format Subpart12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 2005-04. The NACIS Code is 334510 and the small business size standard is considered 500 employees. The Ensite 3000 System with Supplies provides an advanced arrhythmia mapping tool that allows Electrophysiologists to pinpoint the source of an arrhythmia. This BOS announcement constitutes the only solicitation issued. A separate written Solicitation document will not be issued. The system is for VA Greater Los Angeles Healthcare System Cardiac Cath Lab. The Equipment is: Ensite System display workstation, patient interface unit breakout box, mobile cart, two flat panel monitors, printer, Ensitec.5.01 operating software (most current version), instruction of use and one set of RF generator. The supplies include (20 qty of 5 )Ensite Array Catheters and (20 qty of 5 )Ensite NavX Surface Electrode Kit. Include in the price is the standard one-year warranty, training, technical support, and installation. The equipment and supplies shall be delivered to the VA Greater Los Angeles Healthcare System, Warehouse Building 297, 11301 Wilshire Blvd., Los Angeles, California 90073. Offeror shall submit a complete copy of provision of FAR 52.212-3, Offeror Representation and Certification-Commercial Items (Mar 2005). The following Federal Acquisition Regulation (FAR) Clauses and Provisions also apply to the acquisition: 52.212-1, Instruction to Offerors-Commercial Items (Jan 2005); 52.212-4, Contract Terms and Conditions-Commercial Items (Oct 2003); 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items (July 2005). In paragraph (b) of 52.212-5 the following apply: 52.203-6, 52.219-8, 52.222-3, 52.22-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-37, 52.225-1, 52.225-13, 52.225-15, and 52.232-34. In addition, incorporated herein by reference are the following: VA Acquisition Regulations (VAAR) Clauses and Provisions: 852.236-75, Guaranty (April 1984); 852.211-70, Requirements for Operating and Maintenance Manuals (Nov. 1984). Original proposal shall be submitted on company letterhead along with descriptive literature for all items offered to Brian Van Orman, Contracting Officer (NBC/MP). A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Interested firms will be considered only if they are capable of meeting or exceeding the requirement stated.
