Description
There are three (3) purposes for this Amendment #5. The first purpose is to provide an alternate fax number (in addition to the 202-772-9729 fax number already provided) for proposals/quotes. The alternate fax number is 202-205-3204. The second purpose is to correct one of the answers posted in Amendment #3 (with revised language in ALL CAPS below): Can you provide examples (attachments) of the paperwork you require the mediators to fill out? Do you have standard forms that you use for the referral, for the settlement agreement, and the required evaluation? The awardee will provide mediation services to DHS Headquarters as well as organizational COMPONENTS such as Coast Guard, ICE, etc. As such, each organization has THEIR internal form for settlement agreement and evaluation. These particulars will be discussed after the award is made and will not be made available before such time. The third purpose is to answer several questions that were received by the 7/26 3:00 (EST) deadline but were inadvertently not included in Amendment #3, posted yesterday. Those questions/answers are as follows: The Statement of Work says the contractor will ensure that accommodations are available. If a mediation takes place in a location to which the mediator must travel, will DHS reimburse rental expenses or provide a space for the mediation? If the contractor is providing space for mediation will parties to the mediation travel to contractor's office for a mediation? DHS will work with the contractor to mutually agree on a space for mediation. If DHS space is not available, the DHS organizational element will make appropriate arrangements through their internal channels to secure space and notify the contractor. SINCE DHS WILL BEAR RESPONSIBILITY FOR ANY SUCH CHARGES, VENDORS SHOULD NOT INCLUDE SUCH CHARGES IN THEIR PROPOSALS. Will space for the mediation be provided by DHS? If not, will the contractor be required to rent space for the mediation? See Answer Above. What is the estimated number of hours required per mediation? Varies by Case. AS STATED IN AMENDMENT #3, QUOTATIONS MUST BE RECEIVED BY 4:00 P.M. (EST) ON MONDAY, AUGUST 8, 2005.
