Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY SOLICITATION; proposals/ quotes are being requested and a SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation, number N00244-05-T-0864 is issued as a Request for Quotation (RFQ). An electronic copy of this RFQ/synopsis can be accessed at http://www.neco.navy.mil. This RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05 and Class Deviation 2005-o0001 and Defense Federal Acquisition Regulation Supplement (DFARS) current through 20050726 Edition. This acquisition is a competitive and unrestricted action. The NAICS code is 621512 and the business size standard is 11.5M. The fleet and Industrial Supply Center San Diego (FISCSD) requests response from qualified sources capable of providing Symptomatic Event Monitoring Service. Event Monitors shall be provided for patients as needed, round the clock customer service and support and internet access for viewing urgent and emergent patient arrhythmias shall be provided for cardiologist, qty: 15 ea/month. Statement of work is available upon request. The proposed contract will be a firm fixed price contract with a base year and four option years for the Naval Hospital, Camp Pendleton. It is the contractor?s responsibility to be familiar with the applicable clauses and provisions. Contact the Contract Specialist to obtain the full text of all local clauses. DFARS 252.204-7004 ?Required Central Contractor Registration? applies. Lack of registration in the CCR database will make an offeror INELIGIBLE FOR AWARD. Please ensure compliance with this regulation when submitting your quote. Commercial Items (January 2004) applies. This provision contains the guidelines for submission of proposals/quotes. The provision at FAR 52.212-2, Evaluati! on ? Commercial Items (January 1999) applies. The Government will award a contract resulting from the RFQ to the responsible offeror whose offer conforming to the RFQ will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (a) technical capability,(b) past performance, and (c) price. To determine technical capability, (1) offerors must have a web based management database for posting test results that is accessible to the Naval Hospital Camp Pendleton Cardiologist; (2) offerors must have test results available within 5-15 minutes of patient events; and (3) offerors must have a local representative within the Pacific time zone. Those offers that are determined technically unacceptable will not be further evaluated. For factor (b), past performance, Offeror must furnish at least two or three previous contract with the same requirement along with product literature that demonstrate that the of! feror can meet all requirements stated in the purchase descriptions and provide a statement explaining how it will meet the required delivery schedule. Factor (c) pricing, all responsible Offerors are to submit current pricing, and copies of commercially published price lists pertaining to your company?s services that meet the specifications. The provision at FAR 52.212-3 Offeror Representations And Certifications ? Commercial Items, (January 2005) This provision is a fill-in and Offerors must complete all applicable portions and submit the completed copy with its quote.
