- Baynes-Jones Army Community Hospital, Fort Polk, Louisiana has a requirement for Air Ambulance Services (helicopter) to transport neonatal patients to Saint Francis Cabrini Hospital in Alexandria, Louisiana on an as needed basis. Period of performan ce is for 12 months beginning 1 Oct 2005 through 30 Sep 2006 with four twelve month option periods. This procurement is being conducted as a commercial item acquisition in accordance with FAR Part 12. All responsible sources may submit an offer, which wi ll be considered.
