- Description
- Provide storage, delivery, installation, pickup and maintenance of VA-owned Durable Medical Equipment (DME), to Veterans at their residences, within the geographical service area, for the Department of Veterans Affairs, 325 New Castle Road, Butler, PA 16001-2480, in accordance with the specifications and terms of the contract, during the period 10/01/05 through 9/30/06, with three (3) one year option to extend. The Contractor awarded this contract will be responsible for all maintenance, pickup, etc. of DME already issued, and will also be responsible for relocating any stored equipment from prior Contractor's facility to their facility within ten (10) working days from notice of award. The Contractor must meet current standards and be accredited by Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards for Home Care and provide evidence of accreditation with their bid. All questions must be submitted in writing to the issuing office. This is a total small business set-aside. Issue date is August 4, 2005. Bid Opening date is September 1, 2005, 2:00 PM.
