The above referenced synopsis/solicitation has been amended to include the following: PERFORMANCE STATEMENT OF WORK MEDICAL LABATORY SERVICES INTRODUCTION: 1.1. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to perform medical laboratory services as defined in this Performance Work Statement (PWS). Service locations are Grand Prairi e, Austin, San Antonio, and Houston areas. 2. BACKGROUND: 2.1. Periodic physical exams and Soldier Readiness Processing (SRP) require specific laboratory testing for each soldier. Specifically, every soldier requires G6PD testing, and blood typing. Soldiers over forty (40) years of age require PSA testing. 2.1 .1. SB 8-75-10, para. 2-3 dated 20Oct04 specifically states the National Guard will not perform clinical laboratory procedures. Laboratory services in ARNG PESs and TMCs will not be authorized. Operation of these services subjects the facility to Clin ical Laboratories Improvement Amendment (CL IA) certification every 2 years. Supervisors of laboratory services are required to possess a baccalaureate degree in the laboratory services in order to meet CL/A requirements. The costs of purchasing and mainta in laboratory equipment and the requirement to purchase and store large quantities of reagents makes contracts extremely attractive. The Texas Army National has neither the equipment for these services~br a laboratory services supervisor. 2.1.2. G6PD testing is a relatively new requirement for deployment. It is an once-in-a?lifetime test. Soldiers recently returning from basic training are the only soldiers with the test completed. Approximately 1,300 soldiers will require this test within the next five years. Cost should be less than $15 per test, for a total of $195,000 over a five year period. 2.1 .3. Blood typing is also an once-in-a-lifetime in lifetime requirement. AR 40-501 requires laboratory documentation to verify blood type. Approximately 10% of blood types indicated in military records is incorrect. This affects not only the medical fil es, but also the ID card and identification tags. 2.1 .4. PSA testing is required for males over the age of forty during their periodic physical exam (every five years). 3. SCOPE: 3.1. Contractor must pick up laboratory specimens from the exam site in a timely manner, tested in accordance with the Clinical Laboratories Improvement Amendment and results provided to the Texas Medical Command within twenty-four hours of completion of t esting. The contractor shall provide all testing tubes and containers required for each specific test. Contractor shall bill under one account for all services provided within the State of Texas. 4. APPLICABLE DIRECTIVES: 4.1. Clinical Laboratories Improvement Amendment (CLIA) regulation Part 493 5. PERFORMANCE REQUIREMENTS: 5.1. Contractor must have convenient pickup locations within proximately of point of service. Convenient is defined as within fifteen miles, there must be a pick up location provided by the contractor. 5.2. Contractor shall make pickup within 24-hours of drop off. 5.3. Contractor shall provide results of all tests within 24-hours of completion of the tests. Results must be available through web access. 5.4. Contractor shall be certified by CLIA. 5.5. Contractor shall provide all test containers. 5.6. Contractor shall provide one bill monthly for all services rendered. 6. DELIVERABLES: 6.1. Reports of test results shall be available no longer than twenty-four hours after the completion of the tests. 7.
