- The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm-Fixed, personal service purchase order for Psychiatric Services at the Red Lake PHS, I.H.S. Hospital, Red Lake, Minnesota. Contractor is to perform as outlined in the statement of work. Contractor must be available to provide a maximum of 26 on-site visits during a calendar year. The visits will consist of services being provided for a minimum of seven to a maximum of nine hours per visit. Appointments will consist of 20-minute periods or approximately three patients per hour. Services will be provided on alternate Fridays during the hours 8:00 a.m. to 5:00 p.m. Specific scheduling days and times may need to be changed due to unforeseen circumstances. The period of performance will be October 1, 2005 through September 30, 2006. This is a Small Business Set-Aside. Requests for RFQ packages shall be in writing either by telefax or email and it will be made available approximately fifteen (15) days AFTER the date of this publication, which will be Ausust 12, 2005. The closing date of this RFQ package is August 26, 2005.
