Description
THIS NOTICE IS FOR INFORMATION ONLY ? The Health Resources and Services Administration (HRSA) intends to negotiate on a SOLE SOURCE basis with the Center for Aging and Healthcare in West Virginia, Inc. (CAHWV), 517 Market Street, DILS Center, Parkersburg, WV 26101, to support and extended analysis of the data from the demonstration of chronic diseases self-management model to determine if rural/urban differences exists and are maintained over time from rural populations. The analysis concentrates on three chronic conditions with a high prevalence in rural areas: heart disease, functional disability, and cognitive disability. The Office of Rural Health Policy (ORHP) has a mission to improve access, delivery of care, and health status for rural Americans. ORHP has supported prior research that showed that models of care which work well in urban areas do not work as well in rural areas. Sometimes the urban models do not work at all. This is because of the limited number of physicians, specialists, and tertiary care hospitals in rural areas and problems with transportation to care (long distances, rough terrain, poor roads, and bad winter weather). To assure the successful application and continued positive outcomes of chronic disease self-management model for rural America, it is essential to conduct an analysis to compare results over time for rural places to those for urban places. The focus is on three chronic conditions with high prevalence in rural areas: heart disease, functional disability, and cognitive disability. The comparison can identify changes in the model to improve its operation in rural areas. It is important to ensure that the model results in the rural elderly maintaining positive outcomes because of the greater demand in rural areas for care for persons with chronic diseases. This is because elderly persons have a greater rate of chronic diseases and comprise a larger portion of the rural population than they do of the urban population. Furthermore, the Program Assessment Response Tool (PART) process for ORHP has a long range goal of reducing limitations caused by chronic conditions. Because the focus of this analysis is on chronic disease, the results will assist ORHP in meeting its long range PART goal. This procurement shall be processed in accordance with the Federal Acquisition Regulations Part 13, Simplified Acquisitions. The period of performance for this project is 6 months. In accordance with FAR 13.106-1, a Justification for Other than Full and Open Competition is on file. HRSA?s target audiences are particular rural persons with heart disease, functional disability and cognitive disability. Any other organization, other than Centers for Medicare and Medicaid Services (CMS), would have to duplicate the 1994 demonstration funded by CMS mentioned above. To duplicate this demonstration, CMS would have to agree to fund the services out of the Medicare trust fund as they cost several million dollars. Therefore, CAHWV, with exclusive rights to the data, is the only organization that can conduct the analyses requested in this scope of work. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b). Interested persons may identify their interest and capability to respond to the requirement by submitting a capability statement in sufficient detail to demonstrate the ability to perform the services without incurring substantial cost duplication. This notice of intent is not a request for competitive proposals. However, all capability statements received within 15 days after date of publication of this notice will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Reference No. 05-S250-0368
