Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6 and FAR FAC 2005-01. This announcement constitutes the only solicitation, and is issued as a Request for Quotation. A WRITTEN SOLICITATION WILL NOT BE ISSUED. This is a 100% Small Business Set-Aside. This is a firm-fixed price procurement and will be procured using commercial procedures. Solicitation Number is FA4809-06-Q-03012. The NAICS code 624190 applies to this solicitation. The size standard is $6M. PRICEING MUST BE PRESENTED IN A MONTHLY PRICE FOR TWELVE (12) MONTHS. 52.232-18 -- Availability of Funds (Apr 1984) Funds are not presently available for this contract. The Government?s obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer Statement of Work: Air Force Information and Referral (I&R) Program Manager/Volunteer Program Manager 1. INTRODUCTION: The function of the Volunteer Resource Program Manager is to provide active duty and retired military personnel and their dependents, survivors of deceased members, and certain members of the Air National Guard! Air Force Reserve, with information concerning community volunteerism, both on-base and off-base. The Information and Referral (I&R) Program Manager's primary purpose is to implement and administer a comprehensive information, referral, and follow-up system in the FSC, and increase service delivery to military members, their families, retirees, and DOD civilians by enhancing linkages to resources on and off the local installation. The I&R manager will assess needs and determine appropriate referral to on and off base community agencies. 2. QUALIFICATIONS: A minimum of a Bachelor's Degree in Business or a Human Resources Administration/Development- related field. A strong counseling background is preferred. Past proven experience working in an Air Force Family Support Center is highly desired. 3. PERIOD OF PERFORMANCE: 1 OCT 2005 - 30 SEP 2006 4. WORK SCHEDULE: Flexible part time position, Monday through Friday (not to exceed a 32 hour weekly commitment). It will be the contractor's responsibility to ensure that all reports, taskings, and suspense's are met, regardless of a single individual's illness, vacation, and/or professional training. 5. PROGRAM RESOURCES (Provided to contractor at no cost): (1) Office Space (2) Administrative office supplies (3) Computer and printer (4) Forms, publications, & applicable reference materials (5) Access to copy machine (6) Telephone & fax machine 6. DUTIES AND RESPONSIBILITIES: (1) Contractor shall administer the full range of the I&R program and most vital functions of the Volunteerism program according to AFR 211-1 (2) As a measure to increase community awareness, the contractor shall publicize the I&R program and Volunteerism functions by writing articles for publication in the base newspaper and daily bulletin, by designing and publishing flyers, by briefing at commander's calls and other special functions. The contractor shall work with the various base agencies to ensure that publications are accurate, timely, and cover a wide variety of topics to include volunteerism and community resources. The contractor shall coordinate with the Retiree Activities Office in order to inform retiree population of volunteer opportunities. (3) The contractor shall control accountable forms and maintains records to insure accountability. which are in concert with Headquarters ACC. The contractor shall complete forms and reports (using Microsoft Word). (4) The contractor shall receive. evaluate and act on requests for informational assistance using guidelines provided in applicable regulations and personal judgment. Because no two cases are alike, the contractor must rely predominately on personal judgment and interviewing skills to determine actual client needs and referral to appropriate referral agency. (5) The contractor shall coordinate with all base organizations to include commanders and first sergeants to provide them with up-to-date information and briefings regarding community volunteer services and enrichment opportunities available in the area. The contractor shall also coordinate with commanders and first sergeants to apprise them of specialized trends identified within their organizations. (6) The contractor shall conduct follow-up services on referral listings, and with all referral users, to determine if referral services are maintaining quality standards and are meeting the needs of the client. (7) The contractor shall develop consultative relationships with unit commanders and first sergeants to promote the use of Family Support Center (FSC) programs and services. Informally assesses and assists commanders in resolving identified family needs and in developing policies or procedures to decrease work and family stress. The contractor shall provide units with up-to-date information and briefings regarding services or enrichment opportunities available and to identify special needs of families and single members. (8) The contractor shall establish and maintain formal and informal cooperative arrangements with other human services agencies to include military, civilian, local and national organizations to expand the quantity and quality of services available to meet the needs of active duty families. The contractor shall identify family issues and needs and helps integrate the efforts of other agencies to address these issues and needs. Investigates and evaluates, through extensive networking efforts, potential family support referral sources in the civilian community (e.g., counseling services, family development programs, educational opportunities, child care services, legal services). The contractor shall determine whether these organizations or businesses are able to offer services needed at the quality level desired by the Family Support Center. (9) The contractor shall develop referral policies and procedures between the FSC and helping agencies. The contractor shall write memorandums of understanding to formalize agreements and clarify roles. The contractor shall continually evaluate referral sources through reports from personnel referral, and informs agency directors if performance is not satisfactory. . (10}The contractor shall conduct initial interviews with clients to assess needs. The contractor shall provide brief non-psychological assessment counseling and, if deemed necessary, makes appropriate referral to other organizations. agencies, or individuals. The contractor shall provide information to clients about FSC services, requirements, and procedures; interprets related Air Force policies and procedures. The contractor shall answer questions and give information and explanations regarding such areas as employment opportunities. available base or community agencies, and programs dealing with family separation, relocation, or financial difficulties. The contractor shall work closely with unit leadership and other helping agencies to ensure all needs and problems are resolved. (11) The contractor shall implement and maintain the FAMINFO (Family Information) program database. FAMINFO was designed exclusively to assist with Information and Referral activities by authorized users of the FAMNET II telecommunications network. FAMINFO is an automated Information and Referral program, which enables its users to maintain a comprehensive database of information about local and national human services agencies. Data maintained includes both on base and community services such as child care/development activities, medical services, educational opportunities, and civic organizations. The contractor shall maintain accurate and current records in the referral program area. The contractor must evaluate referral agencies and program resources on an on-going basis to ensure quality service delivery. (12) The contractor shall compile, synthesize, and coordinate data for monthly and quarterly usage reports from client contacts to analyze trends and patterns and identify and meet special needs of military families. The contractor shall develop and administer quality control surveys to evaluate effectiveness of FSC programs and services. The contractor shall develop and submit monthly reports via use of the integrated MIS program. (13) The contractor shall develop a volunteer resource program, which will provide a central resource for the recruitment, referral and recognition of all volunteers within the base community. The contractor shall maintain responsibility for the overall coordination and evaluation of the program. The contractor shall coordinate with chairpersons of other agencies using volunteer services in order to facilitate the optimal use of volunteer time and expertise. The contractor shall meet with Commanders and First Sergeants and unit supervisors to determine manpower needs and to develop job descriptions where volunteer assistance is applicable. The contractor ~hall develop and implement an annual base-wide volunteer recognition program during National Volunteer Week. 7. SIGNIFICANT FACTORS: (1) Requires the ability to communicate effectively orally and in writing with service members, families, FSC staff, and representatives of the base and community organizations and agencies. Duties may require working non-traditional hours and/or weekends. Duties may require incumbent to perform telephone stand-by during evenings or weekends. (2) Knowledge of alphanumeric keyboard and the ability to use the computer is necessary. (3) Requires extensive knowledge of problems unique to military personnel and their dependents and the stresses of military life. Background as a military member, military retiree, or military family member is highly desirable. (4) Confidentiality: Must comply with the Privacy Act and maintain confidentiality of customers. (5) Contractor may be directed to perform outreach work at off-site locations assigned by the FSC Director. (6) Contractor may be required to fly by commercial or military aircraft. (7) Contractor may be required to drive government or contract vehicles in Goldsboro or surrounding areas. (8) Contractor may be required to travel in the course of government business and/or perform temporary duty outside the state of North Carolina. Invitation travel orders and reimbursement of travel expenses (IAW the Joint Travel Regulations), attendance at civic and government workshops, symposiums (professional development), conferences related to Volunteerism and Information and Referral objectives: for travel to wing base geographically separated units: and for reimbursement of entrance and activity fees needed to conduct related volunteer and I&R functions. (9) In the event the contractor is ill or otherwise absent, he/she is still responsible for meeting the specifications of this contract. 8. CONTROLS OVER WORK: The contractor works independently of the Family Support Center Director who provides general program guidance and resources. Work is performed independently and with varying degrees of latitude in planning and developing new methods, programming, promotion, and techniques to improve the effectiveness of the center. Unprecedented complex problems and possible solutions are discussed prior to action; written guidelines include Air Force Instructions and Manuals, and applicable policy letters pertinent to the work. The contractor will meet at least monthly with the Family Support Center Director to review the volunteer program. The following clauses and provisions apply to this acquisition: FAR 52.212-1 Instructions to Offerors-Commercial Items; The RFQ should be submitted and shall contain the following information: 1) RFQ number, 2) time specified for receipt of offers, 3) name, address, and telephone number of offeror, 4) terms of any express warranty, 5) price and any discount terms, 6) a completed copy of the representations and certifications at FAR 52.212-3 Alt 1 Offeror Representations and Certifications-Commercial Items (see FAR 52.212-3(j) for those representations and certifications that the offeror shall complete electronically); Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration, (IAW FAR 4.1201 prospective contractors must be registered in the Online Representative & Certification Application (ORCA) at http://orca.bpn.gov ) 7) Period for acceptance of offers - The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation, 8) Late offers - Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is ?late? and will not be considered.
