- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is issued as a Request for Proposal (RFP) SP4410-05-R-8002. This RFP is for the Defense Reutilization and Marketing Service (DRMS) to provide physician services for the Hart/Dole/Inouye Federal Center (HDIFC) employees in Battle Creek, MI. This procurement is a 100% small business set-aside with the North American Industry Classification Systems (NAICS) Code of 621111 with a small business size standard of $8.5M per FAR 52.212-3 and 52.219-6. The resulting contract will be a firm-fixed priced, labor hours contract. All proposals shall include a completed FAR 52.212-3, Offerors Representations and Certifications ? Commercial Items. The following provisions/clauses apply to this RFP: FAR 52.212-1, Instructions to Offerors ? Commercial; FAR 52-212-2, Evaluation ? Commercial; FAR 52.212-4, Contract Terms and Conditions ? Commercial Items. FAR 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders ? Commercial Items and applicable clauses are applicable to the RFP. FAR 52-212-1 (c) is changed to reflect that the offeror agrees to hold prices from 30 to 60 days. Solicitation clause B.3.1, Medical License, requires that contractor be licensed as a practicing medical doctor Internal or Pulmonary (M.D. or D.O.) by the State of Michigan, and provide proof thereof. See clause C.13, Indemnification and Medical Liability Insurance, FAR 52.237-7 for required insurance amounts. Past Performance Proposal requirements are listed in provision E.2. Evaluation of Past Performance is listed in provision E.4. The following provisions require fill-in by the offerors: Bid Schedule, E.2, E.3 and E.5 must be submitted with your proposal.
