- 14 July 2005, Amendment 1, this solicitation is amended in order to provide for a competitive 100% Small Business Set-Aside This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) FA8604-05-R-0177. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24 effective 19 July 2004 and Class Deviation 2004-00001 and DFARS Change Nocie 20040625. This solicitation incorporates by reference the following clauses: FAR 52.204-7, 52.212-1, 52.212-3, 52.212-4, 52.212-5, 52.219-6 and DFARS 252.204-7004 Alternate A. We require a fitness assessment tool that will automatically integrate with health, fitness, and personal training management software. Our specific requirements include: 9 fully automated blood pressure systems, 9 medical body weight scales with an upper limit of 450 pounds, 9 attachment strength bars for measuring upper body strength, 9 skin fold calipers, 9 devices for measuring low back and hamstring flexibility. Additionally, we require 9 cycle ergometers that are 100% compatible with the latest protocols and standards of the AF Physical Fitness Assessment and 9 heart rate receiver clip- ons for alternative treadmill and step aerobic testing. All components need to connect directly to a PC (Dell Dimension Computer: with 17" monitor and printer or equal) and display in real time. Additionally, we require 2-year extended on-site service for the 9 PCs, a 1-year standard service contract for all of the above hardware and software, and installation and training of all 9 systems.
