Description
The Department of Veterans Affairs, Purchasing and Contracting Office (BO-3) located in Tres Rios Building, San Patricio Suite 600, 27 Ave. J. Gonzalez Giusti, Guaynabo, Puerto Rico 00968, is soliciting commercial services for project titled _Preventive Maintenance and Repair Services for Intra Aortic Balloon Pump Equipment located at VA Medical Center, Operating Room, #10 Calle Casia, San Juan, Puerto Rico, 00921-3201_ as described in the solicitation. The Solicitation for commercial services was prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6 and FAR 13.106. This procurement is 100% SMALL BUSINESS SET ASIDE. The North American Industry Classification System (NAICS) code 811219 is applicable with a size standard of $5.0 M. The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 2005-04. Request For Quotation RFQ 672-405-05 is being requested and all offerors must follow the instructions to offerors section, providing their quotations and proposals in writing attention to the Contracting Officer by the offer due date. The Contractor shall comply with JCAHO requirements and maintain competence assessments for the contract personnel selected to perform services for this contract. The contract resulting from this procurement will be awarded to the responsible offeror whose offer, conforming to the solicitation requirements; will be most advantageous to the Government. VA anticipates a single award and a firm fixed price contract based on the contents of this solicitation, therefore, each initial offer should contain the offeror's best terms from technical, past performance and price standpoint. Offerors shall submit all technical questions regarding the solicitation to the Contracting Officer, in writing, no later than 4 calendar days after issuance of this Request for Quotation (RFQ). Questions will not be accepted after Tuesday, July 18, 2005. Questions may be sent via e-mail at Israel.Rodriguez-Perez@med.va.gov or via facsimile (787) 781-8721 attention to Mr. Israel Rodriguez, Contract Specialist. Verbal questions will not be addressed. All responses to written questions, which may affect offers, will be incorporated into a written amendment to the solicitation. RFQ solicitation packages will not be faxed by the offerors. Offerors shall go to BOS web site (www.bos.oamm.va.gov) to download the solicitation package including the attachments and/or amendments to the solicitation. Prior to download Standard Form (SF) 1449 firms must have the FormNet software install in computer. Obtain software as follow: 1) Go to site: www.gsa.gov/Portal/gsa/ep/formslibrary.do?formType=SF . 2) Find SF1449 and double click on form name. 3) Go to Instructions for Downloading Accessible FormNet SoftwareTM, assistance and istallation support is available as needed in this site. In case of time extension of the solicitation due date, the Contract Specialist will be extending the time, via amendment to the solicitation, which all interested offerors shall be responsible to verify the BOS web site, for changes or amendments done to the solicitation prior to the due date. The offerors are responsible to acknowledging each amendment and deliver the amendments to the Contracting Activity Address. RFQ Solicitation package is available now at the aforementioned BOS web site, and offer due date is on July 26, 2005, by 11:00 AM (America/Puerto_Rico time).
