Description
Network 22 Logistics Office intends to award a Firm Fixed Price contract for Annual Diagnostic Radiation Safety Survey and the Departmental Inspection of Imaging Service. The services are to be performed at the VA Loma Linda Healthcare System, 11201 Benton St., Loma Linda, CA 92357. The Contractor shall furnish all necessary and appropriate labor, materials, equipment, and supplies specified herein to perform the services. The contract period shall be for one (1) year beginning August 1, 2005 through July 31, 2006 with the government having the option to exercise four (4) one year renewal periods through July 31, 2010. This procurement is in accordance with FAR Part 12, Acquisition of Commercial Items. The NAICS Code for this procurement is 541690. This procurement is to provide Routine surveys and analysis: Tests and measurements shall be made to comply with United States Department of Health and Human Services, FDA Title 21, Chapter 1, Subchapter J, Sections 1020.31 through 1020.32 (21 CFR 1020), California Radiation Control Regulations (CFCR) Title 17, American College of Radiology (ACR), National Cancer Institute, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO). These Survey Checks and Tests are required by the Nuclear Regulatory Commission and JCAHO to be done on a quarterly and yearly basis. The contractor must be available within 24 hours after repair of current equipment or obtaining new equipment to perform the required tests in order to avoid any patient procedure or study. Service shall be performed at the VA Loma Linda Healthcare System with Dr. Moussa Raiszadeh as the point of contact. Areas to be checked include, but not limited to: 1. Diagnostic X-Ray Performance Evaluation for Radiation Safety Survey, which includes Departmental Review and Availability for Emergency Service. 2. Acceptance Testing. 3. Two C.T. Scanner Evaluations, EE# IS17380. 4. Mammography Unit MQSA Evaluation EE# 15772, which include Mammography Processor EE# 16107 and Mammography Film Viewer EE# 17244. 5. Twenty-Two Diagnostic X-Ray Tube Procedure Performance Evaluations. 6. Sixteen Patient Dose Tube Assessments. 7. Dose Calibrators, EE# 27550 and 17681. One Accuracy Check and Four Linearity Checks per year, totaling ten checks for two machines. The services will be provided between the hours of 6:00 a.m. - 4:00 p.m., Monday through Friday.
