- Furnish 24 hours a day, seven days per week (24/7) Advanced Life Support (ALS) ground ambulance service with 30-minutes response time, as specified in this solicitation for Jesse Brown VA Medical Center, 820 S. Damen, Chicago, IL 60612 to and from Hines VA Medical Center, Fifth Avenue & Roosevelt Road, Hines, IL 60141 and the University of Illinois Board of Trustees. The base period may be less then one full calendar year. This solicitation also includes up to four additional years at the option of the VA. The contractor must provide all labor, equipment, support, and supervision to provide the transportation described in this solicitation. This solicitation is 100% set-aside for small business (NAICS 621399, $6.0 Million). Electronic version and hard copy of solicitation should be available on or about May 20, 2005. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs.
