- (i) This is a combined synopsis/solicitation for the procurement of commercial items prepared in accordance with the format in Subpart 12.6 and Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this no tice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation document and incorporated provisions, clauses and addenda are those in effect through FAC 2005-4 dat ed 8 June 2005 and DFAR Supplement, current to DCN 20050621, dated 21 June 2005. (iii) This Document Number is W91YU0-05-T-0037 and the place of delivery is Bassett Army Medical Center, Logistics Division, Fort Wainwright, Alaska. NOTE: Delivery location i s different from Contracting Office annotated above. Quote shall include FOB Destination shipping costs to Fort Wainwright, Alaska. (iv) This solicitation is 100% set-aside for Small Businesses. (v) The applicable NAICS is 423450 with a size standard of 10 0 employees. (vi) All prospective quoters must be actively registered in the Central Contractor Registration (CCR) at www.ccr.gov. (vii) This RFQ has two (2) CLINS. CLIN 0001 is for five (5) each of Medical Refrigerator, VaxiCool Mobile Freezer, P/N VXC -2, NSN 4110015186521 with dual set points +4 C or -22 C. CLIN 0002 is for one (1) each HemaCool Mobile Blood Storage Refrigerator/Freezer, P/N HMC-MIL-1, NSN 4110015060895. The above items are BRAND NAME ONLY due to NSN Mil Specifications for deployed f ield usage. (viii) Delivery Information: MUST BE delivered to FOB Destination no later than 18 July 2005. (ix) The government intends to award a purchase order to the responsible, responsive quoter whose quote will be the most advantageous to the Gover nment based on price and technical acceptability of items quoted. (x) The following FAR and DFAR provisions and clauses apply to this acquisition and are available in full text at http://farsite.hill.af.mil/ or www.arnet.gov: FAR 52.212-1, Instruction s to Offerors - Commercial Items; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Exe cutive Orders - Commercial Items; FAR 52.211-6, Brand Name or Equal Specific clauses cited in FAR 52.212-5(b) are applicable to this acquisition: FAR 52.222-3, Convict Labor; 52.222-19, Child Labor Cooperation with Authorities and Remedies; 52.225-13, Res trictions on Certain Foreign Purchases; FAR 52.232-33, Payment by Electronic Funds Transfer - Central Contract or Registration; and FAR 52.247-64, Preference for Privately- Owned US-Flag Commercial Vessels (Alt I). DFAR provisions and clauses include t he following: DFAR 252.212-7000, Representations and Certifications and DFAR 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. Specific clauses cited in D FAR 252.212-7001(b) is as follows: DFAR 252.232-7003, Electronic Submission of Payment Requests. (xi) The following addenda are incorporated: FAR 52.212-4 in supplemented by the required use of WAWF (Wide Area Work Flow) to submit electronic invoices to the DoD payment office (DFAS).
