- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation W91364-05-T-0003 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-04. This solicitation is set-aside 100% for small business. The NAICS code that applies is 423450 and business size is 500 employees. This action will result in a FFP purchase order utilizing simplified commercial acquisition p rocedures under Full and Open competition. This requirement is for a direct digital dental imaging system for the dental clinic of the 179 AW, 1947 Harrington Memorial Road, Mansfield, Ohio 44903-0179. The Direct Digital Dental Imaging System is to inclu de two bitewing x-ray adapter apparatuses, and one panograph x-ray adapter apparatus. Purchased dental imaging system will be adapted for use with the current dental x-ray equipment, which is: (2) Bite Wing Dental X-Ray units, Manf: Dentex Corp, Manf Date Jan 2004, Manf Model Number: 8461-406602 (1) Panograph X-Ray unit, Manf: J Morita USA Versaview Panograph, Manf Date Jul 1987, Manf Model Number: AZ X-Ray Assembly. If vendor is unable to quote retrofit to existing panograph X-Ray unit, then include in your quote substitution of new direct digital panoramic x-ray unit. DICOM File format storage required. Two (2) intraoral sensors equivalent in size to #2 intraoral x-ray film, with universal hold er sets for each sensor required. Vendor will be required to provide installation, and once installed, provide formal training to all dental medical radiology personnel. Kodak RVG 6000 or equal. The Government will make an award to the responsible offeror whose offer conforming to the solicitation is the most advantageous to the Government. The following shall be used to evaluate offers: price or cost to the Government, time of delivery and service response time.
