Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement is posted as a request for quotation (RFQ). A set-aside does not apply to this acquisition. The Government requests procurement of Linatron M9A Dual Energy Modular X-Ra y Linear Accelerator (Programable Logic Controlled (PLC)). Included is customer defined dose rate and energy, power selection (to be determined), customer factory acceptance test, remote control collimator, motorized manipulator, laser, manipulator table, transformer, voltage regulator, constant voltage assembly, indoor/outdoor temperature control unit. Include tuition and materials for one person for Linatron Support (maintenance)course for 4 days is required. Offers must propose pricing, availability a nd description of offered product, plus training as described. Offerors must include a completed copy of the provision at Clause 52.212-3, Offeror Representations and Certifications, Commercial Items. Applicable clauses are 52.212-4, Contract Terms and C onditions, Commercial Items, Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items. Evaluation factors and relative importance of those factors are in accordance with FAR Part 13 which states t hat offers shall be evaluated in accordance with the technical capability, price and past performance. Response to this RFQ must be signed, dated and received via electronic mail or FAX by July 7, 2005, no later than 10:00 AM EST at the US Army Contracting Agency, SFCA- NR-APC-T (Jo Ann Hayes), 4118 Susquehanna Avenue, Aberdeen Provi ng Ground, MD 21005-3013. Vendors not registered in the CCR database prior to award will not be considered for award.
