Description

The Center for Health Care Contracting, MEDCOM Health Care Acquisition Activity intends to pursue a Single Award, Indefinite Delivery/Indefinite Quantity, Personal Services Task Order Contract to provide EDIS Services for various Army CONUS Locations at Ft Benning, GA, Ft Campbell, KY, Ft Rucker, AL, Ft Stewart, GA and Ft Bragg, NC. This contract is intended to be awarded for a base and four (4) option years. The contractor personnel shall serve DOD eligible infants, toddlers, school-aged children wh o have developmental delays or disabilities per the DOD criteria, and their families who are assigned to duty in domestic locations supported by DOD schools. Total number of children/families served is approximately 1,000. Personnel categories include ped iatrically trained health care providers, including Developmental Pediatricians, Psychologists, Nurses, Occupational Therapists, Physical Therapists, Speech-Language Pathologists, Early Childhood Special Educators, Certified Occupational Therapy Assistants , Physical Therapy Assistants, Child and Family Specialists, Social Workers, Assessment Technicians/Specialists, and Assistant Family Service Coordinators. This procurement will be a 100% Small Business Set-Aside, Request for Proposal. For information pur poses, the NAICS code for this acquisition is 621340 and Small Business size standard is $6.0 Mil. Solicitation W81K04-05-R-0029 will be issued on or about 22 Jul 2005 with a closing date of 30 days after issue date.
