- FSC Code: 65 Title: Tube Sealer NAICS web site: 339112 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is 797-FDF8-05-0020. This solicitation document incorporates provisions and clauses in effect through FAC 05-04. This solicitation is 100% set aside for Small Business. The associated NAICS Number is 339112. The Air Force Medical Logistics Office has a requirement for the following item(s): Quantity of 5 TSCD Sterile Tubing Welder, Part Number: 3ME-SC201ADP, each to include: 1 year warranty, 1 cartridge of wafers (70 wafers), 1 wafer disposal bin, 1 power cord, 1 operating instruction manual; Quantity of 5 cartons , TSCD Welding Wafers, Part Number 1SC*W017, Quantity of 5 each, TSCD Bag support shelf, Part Number 8ME*TSTAND, Quantity of 1 ea TSCD Operating Instructions Manual, Part Number SDIN004 . Terumo Medical Corporation is the manufacture of this equipment. Delivery, which shall occur within 60 calendar days of contract award, shall be FOB DESTINATION to the Travis Air Force Base CA 94535-1800. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-2, Evaluation ? Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: Award will be made on Brand Name or Equal as well as Price. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. All contractual technical questions must be submitted in writing (e-mail or fax). Telephone questions will not be accepted. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. Offers are due by 12:00pm EST on 05 July 2005 at VASS.
