- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W9124V-05-T-0015 and is issued as a Request for Quote (RFQ). The NAICS code is 334310 and the size standard is less than 750 Employees. The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular 2005-03, Effective 11 Apr 2005 & Class Deviation 2005-o0001. The proposed contract is 100 percent set aside for small business concerns. The acquisition is a firm fixed priced purchase order in support of the Montana Army National Guards public Affairs Office (PAO). PAO requires a CD/DVD duplicator-printer with dedicated PC and initial load of supplies. The propose of the system is to affix printed labels to CDs & DVDs to prevent the physical label from being perfectly centered on the disc which can result in an unbalanced disk. The duplicator shall consist of the following: One (1) ElitePro2 DVD Plus/Minus R with Autographi6, five hundred (500) DVD-R DM ultra silvr Inkjet 8x, one (1) 17 Standard Monitor, one (1) ElitePro2 DVD plus/minus R with autograph6 printer, one thousand (1000) Disc Makers Ultra CDR Silver inkjet Printable 52x, two thousand (2000) Retail Super Slim 2 pc CD Box Clear, three (3) 18 L0032 and three (3) 18L0000. For descriptive purposes, the equipment shall be brand name or equal to Disc Makers products. If the offeror is quoting an equal item, please provide technical specification and warranty information. The provision of FAR 52. 212-3, Offeror Representations and Certifications-Commercial Items (Mar 2005) applies to this RFQ. 52.212-3, the clause needs to be completed and a signed copy of this provision shall be submitted with any offer. 52.212-2, Evaluation, Commercial Items ( Jan 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors s hall be used to evaluate offers: Price, Past Performance and Technical Capability. Past performance and technical capability, when combined, are significantly more important when compared to price or cost. Vendors are requested to submit past performance information to include persons of contact on their last two contracts for similar products.
