- The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items to provide 1 Orthopedic Surgeon and 1 Orthopedic Technician required for Government beneficiaries at Mountain Home AFB, ID 83648. Contractor care shall cover the range of services provided in a civilian medical treatment facility. Performance shall be according to the requirements contained in the performance work statement (PWS) and professional standards of the Joint Commission on Accreditation of Healthcare Organization (JCAHO). The solicitation will result in a firm fixed-priced contract with a base year and four one-year options. Base Year Period of Performance will be from 1 Oct 05?30 Sept 06. The North American Industry Classification System (NAICS) code is 621111. This action is being advertised as a 100% small business set-aside. A formal Request For Proposals (RFP) will be issued on or about 10 July 2005 via Fed Biz Opps. Any prospective contractor must be registered in the Central Contractor Registration (CCR) (http://www.ccr.gov) in order to be eligible for award.
